Shodan from the first System Shock.
I may be paraphrasing here, but here is the gist of what she said.
"... when my cyborgs put you into an electric interrogation chair, I will learn your secretssss. And you will learn more about p-p-pain that you eeeeever wanted to knooooowww."

That phrase makes my neck hair stand end-wise. The last sentence where she draws the phrase out in a low pitch, slow-motion is really creepy.

The last line of a Bradbury short story, The Whole Town's Sleeping I believe it's called

Basically this girl lives in a town where a mysterious man has been going around hacking up young girls out on late night walks. The girl walks home from a movie with three friends, of course they live in succession and of course she's the last one home. After running full bore home scared out of her mind, nothing happens, she shuts the door breathing a sigh of relief at how stupid she'd been the whole time. The last line of the book is the creepy as hell part.

Something like this; (and it's only a synopsis since I can't find my anthology to quote directly right now)

She shuts the door, breathing a sigh of relief at her foolishness in believing in such foolish tales of murders and such.

From behind her a man clears his throat.

-Gives me chills every time

Gears of war 2 in the haunted house level (You think of a better name) when that computer kept saying things like "Here at new hope, cleanliness is next to godliness" before deploying turrets up my ass. Bioshock was wry with good, creepy quotes, one in particular was from that wicked old surgeon Steinman. "Steinman, she said, I have what you're looking for. And then she cut me inot a thousand beautiful pieces". Also in Dead Rising that clown guy was scary as fuck "(Crying while laughing) Then everybody started to die (Sniffle) BWUAHAHAHAHAHAHAHAHA!".

The first explosions tore open the entire medical section. All my work, gone. I was trying to understand how it could've happened when I saw him, the man from room five. He looked at me, not with eyes, there were no eyes, but I know he was looking at me because I felt it. Oh, God, what have I done?

V for Vendetta

also: "No, they're dead. They're all dead...(grins and looks right at the camera) except for me" -Alex Mercer

finally: "Every species can smell its own extinction. The last ones left won't have a pretty time with it. In ten years, maybe less, the human race will just be a bedtime story for their children. A myth, nothing more." -John Trent

Thinking back on it...

"My mother risked her life for me, and now you too...I should have saved you. I should have been the one to fill your dark soul with LIIIIIIIIGHT!"

In the beginning of bioshock, when you are hunting the plastic surgeon and there's the quote "Above All, Do No Harm," written in blood on the wall. For some reason it just scared the crap out of me. I stood looking at if for like 30 seconds before I could tear my gaze away.

I'm actually surprised no one has said this yet:

http://www.youtube.com/watch?v=oywfsv2SJNg one of the most disturbing conversations ever.

Specially the line "Monsters, they look like monsters to you?" -Vincent, Silent Hill 3.

Kill a man, you're a Murderer.
Kill one thousand men, you're a conqueror.
Kill all men, you're a god.

not a video game, Flavour text from Wrath of God.

Sorrin's quote from Day of Judgement is pretty good too.

When I first found Rat Man cubbyholes on the first play-through of Portal I was nicely creeped out. Every one of them had something extra to make it creepy.

Prototype where the main villainess says:
"I am your mother."

I found this creepy because I found the character hot and attractive regardless of any possible infections to come. So when she said it, it got kind of awkward and let's just say I had no qualms about hunting her down.
That's really weird. I always found her repulsive, what with the hair & the trickle of blood on her chin & the TURNING INTO A GREAT BIG RABID VAGINA.
The internet does this to a man, if you go far enough down the rabbit hole. Here's a tip, take a left after the Bazaar of Discontent and you'll find yourself at this wonderful bistro, order the lobster. Great stuff.
with butter and tomato soup?
Tomato soup is great, but if you can't have tomatoes due to an allergy of some sort. The butter is good, they say they use the salt of crying angels as they churn it. What makes the angels cry? Why, what they see on the internet and [insert obligatory masturbation joke here].

Bioshock also has loads, it's in the atmosphere. A lot of people seem to think that, won't stop me from saying the first creepy moment I saw. That woman who upon the very beginning is singing to her baby. I found it creepy because she adopted a pistol, well... I made it burp at her.

My memory is shot (Ooo, a gun pun. Hey! That rhymed.) during these kinds of things and as soon as I remember better I'll share some more. It's just not a whole lot is actually creepy to me, but dancing pink bunnies with chainsaws that churn fire instead of diamond tipped blades and they're all singing Barney's I Love You song as they come towards you with their horrifyingly disfigured faces and damaged torsos... That would freak me out a bit, but then I'd get over it and a new bar is set.
