Okaaay that's...a bit, out of the ordinary. You could always go with those multisided dies used in dungeons and dragons, then again I've never played so I wouldn't know.

For my birthday last year, one of my players (I am the DM) made a gazebo cake (complete with demonic eyes and fangs) in the same style as the picture from the OP as in the famous tale of Eric and the Gazebo.

Beyond that, I don't think you can get any more iconic for D&D than a beholder.

The difficulty with a beholder cake is that beholders are pretty specifically ugly. While I'm not expecting the cake to be pretty per say I want people to want to eat it.

Would it be fun to decorte cake out with the favorite food of your best friend and closest family made out of chocolat or marzipan? It would look like a cozy mess of food and your best friends and closest family would feel appreciated and everyone else will think the cake looks cool because of all the details. Don't forget to dedicate the upper level or middle of the cake to you and your loved one, because that day it's all about you two.

I think a great cake would just be the game board itself. A grid pattern with various models, props and dice on top.
