actually the most i've ever played a game is the one i'm still playing daily: skyrim! i'm at 150 hours right now.
I'm pretty certain I've played through Paper Mario no less than five times, so that one is pretty high up there. I played a lot of whatever shooter's I owned at the moment when I was younger, so a couple James Bond games and Halo 2 are pretty far up there as well. Beating Fire Emblem POR a few times probably puts it up pretty high on my list too.
I clocked over 2000 hours on Halo 3. For several months that game was all that i would play.

Fallout 3 or New Vegas, probably more into New Vegas though, I don't have an exact count but it was probably around 120 hours, it is kinda crazy but it's just free time, other people fuck around with Facebook while I fuck around with super mutants.
A few years ago I clocked 165 days played in WoW on the characters I hadn't deleted. I gotta be well over 200 days these days. So much time wasted...
As for single player games I think Final Fantasy VIII is my record holder. Don't have any exact numbers, but I've played the game over and over so damn many times.

I'm pretty much bethesda's ***** right now.
Skyrim 253 hours, FO:3 500 hours, and FO:NV 400 and change. I can guarantee I'll be declared missing when Dishonored is released.

That would be Street Fighter 4. I stopped counting because I've switched versions and consoles, but on my old Steam account I had 300 hours, which I played on two years ago before getting a PS3. I have probably ranked up 1500+ hours by now, and I still play from time to time.
It's hard to say. I have clocked around 650 hours in Shogun 2, probably far more in EvE online.
Really though, I spend most of my time while gaming surfing the web simultaneously or writing papers... combined with the fact that I have a penchant for letting games run for days in the background without stopping them until they inevitably take up too much RAM. This means I might play 2 hours in a day where the game was on the entire 24.

I've clocked in about 400 hours on Pokemon Ruby. It's my first Pokemon game and I love it to bits.
