Those guys are pretty damn difficult to fight at close range, my strategy for them is to keep my distance as much as possible and throw fireballs (fireball, fire orb, great chaos fireball etc) back at Ornstein until he dies, then fighting super Smough is pretty straight forward and he is pretty much like another Asylum Demon but with lightning attacks so try to dodge him instead of blocking. You could take out Smough first if you want but I find super Ornstein to be a much harder fight as he is very mobile. Basically just never ever let the two of them be upon you at once, if they get too close or corner you then stop locking onto them, roll out the way once or twice and then sprint far away. You are going to want to be under the 50% equip load, if you wear Havel's Ring and the ring of favor and protection you can achieve this while wearing stone armor or +5 giant armor without needing your endurance to be too high. I've never bothered with Iron Flesh, probably save the attunement slot for one of the combustion or fireball spells.

The Four Kings was the only big thing that really got me more than it did the first time around. They were annoyingly hard on NG+, especially as a slow moving heavy hitter.

Im actually having alot of fun on my first NG+ with my min/max mage. Most of the bosses are pretty easy... but screw Manus. Im serious, all of the AotA bosses are brutal to low vit characters, especially on NG+. Just to dang aggressive.

I didn't have much trouble with Sen's Fortress, I pretty much know the place off by heart but if you want some advice try using pyromancy, it helps a lot because you can take out those pesky snake people at range and then make your way through the swinging blades stress free. Also the boss doesn't have much of a fire resistance either from memory. Hell, pyromancy pretty much solves all your problems in Dark Souls, I didn't even bother trying to parry Gwyn, just circled him spamming combustion and great combustion etc and killed him very quickly.

Back when Diablo 3 was a pretty fresh game, Act 2 on Inferno was a very frustrating brick wall for the very under geared me. Not much fun at all.

If you are patched up to date then Iron Flesh is nerfed beyond belief to the point of being almost useless. It needed dealing with though as it used to be too godly against a lot of bosses and was pretty much a cheesy way to brute force yourself through the game.
It's not nerfed beyond belief, it still gives you a big defense and poise boost, just one that's more conducive to an actual tank build, without making every single character an invincible badass.
Well that's a pretty huge nerf depending on how you look at it. If you are running a tank build I think it's more important to maintain the small amount of agility you have. IIRC it also lowers some elemental resistances so its usefulness is incredibly situational post patch. In fact I struggle to think of a fight I would use it in.

For a non Dark Souls example, World S in Super Mario Galaxy 2. First three levels? Nothing bad. The rest, including Green Stars and Grandmaster Galaxy? Rage has been induced to hell and back.
I always found taking off all my armour and putting on a good shield and black knight axe worked best. I never had a problem with them and always killed them before the next spawned. Just beat them down two handed and switch to a shield to block their attacks. You can easily roll out of the way of their aoe.
