I play all of my games on the Normal diffculty (With the exception of MW2, that I played on Veteran first). Afterwards, I either replay the game on Easy if there are items I have missed.

The only games I ever up the difficulty on are Tekken and Soul Calibur games, which I usually play on the hardest setting.

Conversely, I probably never would've played through Saints Row 2 without the casual difficulty setting. I'm not very good at those kind of games, so having the easy setting meant I could enjoy the story and humour without getting pissed off.

I start on the easiest, cause I want to take in the story of the game, then I'll do the hardest difficulty to see if I'm still up to it.

Normal on the first playthrough, then progressively harder difficulties on later playthroughs.

Except Dragon Age, had to drop down to Easy on that. >_<

When I start a new game, I tend to just play it on normal difficulty, then I will go for the higher difficulty settings.
As for difficulty on my 3 favourite games...
Whatever I feel like at the time.

I play easy to get the story and then play other difficulties if.
A: It is a fun game and i want to play it again or.
B: There is some cool stuff to unlock on other difficulties.
I play KH2 on hard SSBB on all difficulties and Castlevania SoNT on the default difficulty.
