Always start on normal, and Hard if I enjoy it enough to play again. Metal gear solid 4 was vastly superior on the second hardest difficulty i found, the boss fights are terrific when you actually fight them. But for a fighting game I'll usually never up it because I suck at it. I still replay half life 2 on normal, I know hard would just piss me off, because I'm not great at shooters.
There's only been a few games I'd confidently play at the hardest difficulty to enjoy and not simply for challenge and that's F-zero gx and the tony hawk's games. If you don't play f-zero on master you're missing the best racing game ever, it's really fucking intense.
