End of Evangelion (the movie which is supposed to end the Neon Genesis Evangelion tv series) WHETHER YOU HAVE SEEN THE ANIME OR NOT!

Seriously, it's incredibly strange and weird, fucked up, horrifying, almost nauseatingly so. it's even worse if you like any of the characters (I find them all repulsive human beings). It's a film you won't forget, and unlike anything you'll ever see. Add to that, I don't think it's a good film either.

It seems you've heard about/seen most of the ones I was going to mention, so I guess I'll say the following, although I've only seen the first thing. Others I've just heard about.

The Wicker Man (original, the remake is unintended comedy)
The Devils (hard to find though)

Human Centipede, Full Metal Jacket, Falling Down, Saw, Requiem for a Dream, The Machinist, have already been mentioned but deserve repetition.

I would add Tetsuo the Bullet Man, Dagon, and Meatball Machine to the list. Meatball Machine might not qualify as disturbing, depending on your threshold, but it is a ridiculous gorefest with scenes of tentacle rape and eyeball drilling. Dagon should be seen for its oppressive atmosphere alone, the pervasive wrongness it conveys so well.

Captcha: look about. Yes, captcha. When I've loaded somebody's suggestion onto my Netflix at two in the morning with all the lights off........Look about, indeed.

Underground is a comedy. A black comedy, yes, but still a very funny movie. Watch it with Black Cat, White Cat for good times.

Indeed. Although kids today might not think so having never lived under the looming spectre of mutually assured destruction.

I came to say Come and See because as disturbing as it may be it also means something and is very far from the exploitative trash that is so often regarded as disturbing. Almost as disturbing is Die Brücke especially when you consider it is based on real events.

OP hadn't specified what kind of disturbing, and there is enough black in Underground to qualify.

But yeah, Black Cat, White Cat is cool, too. :)

Jesus Camp, it was the only movie where I actually sat there wide-eyed, jaw-dropped, hand over mouth, cold sweat, in complete disbelief that people could be so fucking crazy and even worse, pass that craziness on to their defenseless children and turn them into mindless little slave zombies like them. The host of that camp needs to be killed.

Taken was kind of disturbing, only because that stuff happens everyday, even as close as in the states. The movie itself was fantastic and well-done but the content is what messed me up.

And The Grudge 1, Kayako has always terrified me lol.

Trainspotting is disturbing just because of how realistic it is, although maybe it's not disturbing in the way you mean it.

I'm a little surprised this hasn't been name dropped yet, but Perfect Blue was pretty damn creepy for me. It's basically an anime version of Black Swan but is somehow darker and more insane. Part of that comes from the confusion as to how much is really happening, or is just being imagined. Sure Black Swan did that to an extent, but its way easier to tell in that film when the character's just tripping balls. Apart from a few moments, it's kinda hard to tell in Perfect Blue, though a lot of the stuff that actually happens is still unsettling anyway :/

Really weird ending though, and not in way you'd expect...

Totally black and white, (thanks to almost a year of post-production), no dialogue, or music, just sounds of the wilderness and some of the actions on screen. It's like a surreal nightmare captured on tape.

It would have been better as a shorter film, but if you want something disturbing, check it out.
Not a lot of people seem to know about the film...I'm not sure if that is a good or a bad thing.
Oh, fucking Begotten, man. I forgot about that freaking film. What the fuck happened?

Turn the language to French and turn on the subtitles, and it becomes a French Surrealist Art Film. Also, the French Freddy has a hilarious voice, beyond belief.

If you're looking for a good disturbing movie, check out Super. One of the best films I've ever seen, and basically shot on a shoestring budget.

If you're more into pure shock value, I'd say go for the infernal Threesome, i.e. (in ascending order of disturbedness) The Human Centipede, A Serbian Film and The Human Centipede 2. Although I've kind of found it hard to be too disturbed because everything in those movies is just so over-the-top.
Still, I would say that A Serbian Film actually has some appeal beyond the pure shock value. As the director basically said, it's a representation of how, as a Serbian citizen, you're basically getting fucked from the cradle to the grave, and the main character's progression mirrors the role of the average Serb in the Balkan wars. At any rate, the atmosphere it generates through music and visuals alone is extremely impressive.

This whole explenation of the movie is pretty much bullsh*t, and feels like they came up with it, just in case anyone critized it as just torture porn. (I myself hate that phrase, but it really applies here.)
If the struggles of serbians were really the issue, they wanted to capture, they could have had a protagonist, who is say a factory worker, who loses his job, and struggles to provide for his family, so they don't have to live out in the streets. That could have been an interesting drama, that perhaps a lot of people could relate to ... but hey than it wouldn't be known as one of the most disguisting piece of movie making ever ...
The equivalent would be, if someone wanted to show the hunger situation in Ethiopia, and make a film filled with cannibalism, bestiality and murder .

I will add Uwe Boll's movie 'Rampage'. It is actually decently directed which I know is heresy to say in many circles. It is about some college aged guy who makes a suit out of armor and goes on a killing spree in his town. There is social commentary sprinkled throughout about mass media. If you only know him from his cheesy video game movies, check this out.

Also almost anything from Takashi Miike will suffice. 'Visitor Q' made me never want to watch a horror movie again, because how sick it was. "Ichi the Killer' is disturbing, but at times fun because the yakuza guy acts like a crazy, masochistic hair metal guy. Then there is that famous one of his that makes Stephen King's misery look pure and innocent. The name escapes me.
