I think Left 4 Dead has more potential for accidental douchebaggery than any other game. So much so, that intentional douchebaggery just isn't that fun. Valve got it just right.

I was playing Team Fortress 2, and my team was attacking on Dustbowl stage 1. We'd capped the first point, and several of my teammates were preparing an assault behind the one-way door that leads to the second point. Unfortunately, An enemy engineer had constructed a sentry on the other side, blocking their progress. I come along, as a spy, and decide to lend a hand by waltzing over and dispatching both the sentry and the engineer. I would be a hero. I disguised myself, cloak, and step through the door, completely forgetting that the sentry would not hesitate to target the men behind me. I was swiftly eviscerated, along with two of my comrades.
Just last night I was playing Left 4 Dead, versus on No Mercy. The other survivors and I were chasing down a Smoker. I finally get into a good position to fire off a shot, and I do so, hitting it square on with my shotgun. However, I also happened to hit the gas station directly behind it. The resulting explosion kills one of my friends and incaps the other two. On the bright side, I escaped completely unharmed.

Playing search and destroy on COD5, aiming for the enemy with my signal flare, hits pillar and lands in the middle of our team. We get raped in our blindless. Shouting ensued.
First time I played Team Fortress 2, I just ran around shooting my teammates with the Syringe Gun, thinking that I was helping. Thank Christ I didn't have my headset plugged in.
Once when playing R.O.S.E Online beta with my best friend I did a "zulie hack" from his avatar - as a joke I was going to leave him with 69 left, instead I took every single zulie coin that he had - but it dropped around him, 1 zulie at a time, by the time he had collected 10k back the server disconnected and he had lost about 200k XD
And once when playing a WoW private server (my friend was a cheapskate) I convinced my friend that there was a secret island in the middle of some lake - but he was a very low level and he got instant killed when he went near the lake and couldn't get to his body and away in time.

Epic Battlefield 2 Fail.
Wake Island. Flying a transport chopper full of GIs. Chinese Jet flying at me. Tried to jink sideways. Lost control. Crashed chopper, not into the sea, but straight on to a transport boat full of people underneath me. Pro TK skills.
