My favorite twist requires a bit of a setup for its weight to hold meaning.

In a not so well known game called Second Sight, you play as the psychic Dr. John Vaatic who suffers from what appears to be amnesia and wakes up in a present day medical lab where the intent is to experiment on him because he's a psychic. During his escape, he begins to have weird flashbacks when he finds out he was on a cold war op named Project Winter Ice. When he discovers that one of his teammates was KIA he has another flashback in which she does not die, which apparently changed history and now she was alive but crazy. Few more plot filled flashbacks it becomes apparent that he is changing history to suit his needs except that entire time

the "present day" is actually in the cold war and he's looking into the future outcomes based on his actions in the "present" without realising it.

To this day the fact that they pulled the wool over my eyes for so long just blows my mind.

But seriously, as far as plot twists go its nothing spectacular, but the one that always got to me most was in Dragon Quest V.

After you go into a dungeon to recue Prince Henry, your father Pankraz comes in to save your ass. Bear in mind, at this point you're 6. When you leave you're assaulted by Ladja, a skeletal bloke in a bishop outfit. He kidnaps you and uses you to make Pankraz stand down, killing him right in front of you. Then you get sent off to slave camp for ten years to kick you while you're down.

That's also my pick. That was the best plot twist in any game I've ever played. It left an impression on me long after I had finished the game.

I gotta say, I was actually pretty surprised by it. Should have seen it coming, but they did a really good job of covering their tracks.

Jenny gets kidnapped, but the Darkness doesn't let you rescue her, instead making you watch as Paulie blows her brains out

Unlike most you will find on these lists, this one actually happens fairly early in the game, but its a huge sucker punch to the player, and the moment where the game switches tone from "Gangster with superpowers" game to its dark revenge tale.
