Well as a native of the NY/NJ area the only way to eat pizaa is to pick it up, fold it in half lengthwise and go to town, crust is eaten last as it is the main support structure. If a pizza requires a knife and fork then it is no longer pizza.
This. That's the way to eat a pizza. Heck, it's the only way to eat a good pizza.

And here I thought the Escapist was full of intelligent people... did somebody really just quote Bill O'Reilly in a positive light?

OT: Easy answer: Depends on the sort of restaurant I'm at. If it's a take-away, with my hands*. If it's a swanky restaurant, with a knife and fork. Always pointy end of slice first.

*If the pizza is in 6 slices or if I need to slice it myself, I will slice it into 6 and take alternate pieces so after 3 slices I can have the radioactive symbol made up of pizza slices. In my head I am an eight year-old.

With my hands, I find using fork and knife weird.

What bothers me with the way other people are eating... eating with your mouth open, it just disgusts me.

I stick it in the blender with a chocolate milkshake.

Then I drink the result.

Depends on where I eat it. If I am at my friends or at home, I eat it with my hands. If I am at a restaurant I eat using a knife and fork if they provide me with one.

I hate people eating with their mouths open. I don't want to see half chewed food thank you very much.

I like to do it in front of the TV with friends and a beer, using only one hand to insert it into my mouth.

Finger food in most cases... I've only used a knife and fork on BJ's Chicago-style pizza.

Holy shit, is that some good, deep-dish pizza.
You ever Had Giordano's Deep Dish? I promise you, you'll never look at thin crust the same way again.

OT: Depends. If I have Giordano's deep dish, I praise the Pizza Gods first. Than I use my hands. Anything else I fold in half and eat.

Megalodon: French/Italian pizza? I was under the impression it was simply italian. Maybe italian-american depending who you believe.
The original Neapolitan-style pizza they make in Europe and certain fancier restaurants (usually authentic Italian as opposed to Italian-American) in North America is supposed to be eaten with a knife and fork. There's an Italian place in my neighbourhood with a stone oven that makes this kind, and I use knife and fork when I go there.

The fast-food pizza you get from a typical North American pizza joint, I eat with my hands. I'll usually take two slices, put one on top of the other sandwich-style, and chow down.

If it's a big, messy pizza I usually eat it with a fork and knife, if it is one of those grocery store pizzas that you put in the oven and tastes like cardboard, I eat it with my hands.

Eat with hands.

About half way up, then I take a big bite of the crust on either side, as it's been shortened, it doesn't flop everywhere, and I now have more Pizza than crust

Sparingly.

Other than that: with a fork. I hate the feeling of greasy hands, unless it's from fixing or building things.
