They tell me that next time I die, I have to leave, so I decide that this time, Tyson ain't gonna get me. I knuckle down, kick Mike's ass, grab the sphere, run back to save...and fall through a gap in the floor and die.

Flashbangs. I threw one that just didn't quite make it through the doorway. As I'm about to be blinded, I start towards the door, priming a grenade. I get blinded, throw the grenade wonky so it sticks to the doorframe, the guy I was trying to get to in the room shoots me to fuck, my bullet riddled corpse falls to the ground, only to be blown up by my grenade stuck to the doorframe.

I used to suck with flashbangs.

I had the exact same thing happen to me when I first played Deus Ex. I still remember painfully draggin myself along the corridor of holograms.

I think ive managed to do that a dozen times now.... almost every time i play it i screw up holding the TNT XD

We were playing Team Doubles in Halo 3. We were playing 2 players, 1 of them killed me, the other one got into hiding already, being followed by the player who killed me. Long story short, we were crouching across the map and after 2 minutes we had found their spot, so we waited for the clock to hit the 10 second countdown, threw nades up there and won the game 2:1.

Dude, Fallout 2 kicked my ass from Shady Sands to Rivet City and back again on easy difficulty. That game is bastard hard!

I was playing Red Dead Redemption, and i had just saved some law guy outside of Armadillo, so when i headed into the town, i forgot how to stop my horse and smacked into the same guy i had saved and several others.
I went to jail for KILLING him.
He died.

Battle field 2: Gulf of Oman, playing as a sniper with a mate of mine. we were sniping on a roof top and claymored the ladder. A guy parachuted in from the helicoper and started a knife fight with my mate, as they are fighting my mate try to jump off the roof and gets blown up by my claymore.

Yes, that is usually the outcome of killing someone...
I know what you mean, it didn't make much sense but i couldn't be bothered to change it.
I ran over the guy with my horse by accident, he died, i got jailed for killing a police officer.

I'm not a big fan of FPS games, but I did get into MAG for a while. Still, I've had a few Epic Fails. The worst would be throwing a grenade down a hall, then rushing down the hall and getting killed by my own grenade. I've done that a few times...

I loved Arkham Asylum, but I remember during the Killer Croc encounter I didn't know about throwing a batarang to knock him into the water, so I'd get so far and then he'd jump out. I would run all the way back to the start. Did this a few times. When he finally caught me, the continue screen told me how to knock him into the water. All I could think was: "Where was that piece of info earlier?"

I was playing FEAR 2 on one of the levels with those crawly assholes.The psychic commanders that went wrong.Since I had to eat before work I went to get my meal and eat as I play along.But weirdly I put my headphones on and started playing.My bro who was next to me said that I don't need them so I took them off but I didn't notice I hadn't paused the game.One of those psycho's crawls out of the woodwork and starts slashing me.I panicked so hard I started yelling and I couldn't hold the mouse properly to even kill it.This is all considering I've played the game previously and knew the fucker would come otu there.Twas a major fail and even worse my brother lol'd his ass off right beside me .

Wow, both of the locations you mentioned weren't even in Fallout 2.
Also, it isn't THAT hard. You just need to start carrying NPCs and Automatic weapons.

Mine would be from Frozen Synapse.
I was rocking hard with my team on a multiplayer match just when I "accidentally" fired my rocket dude's rockets at my whole team's feet.
I failed so hard that I can still feel the pain.
