Making enemies do more damage with the same attacks.

Surely there are better ways to make a game difficult - like require strategy, perhaps?
this. between fucking fighting games to shooting ones, please to god, just concentrate on the AI's IQ, and then we might be able to get somewhere without having one hit kills around every corner.

ot:

# Bladder of Steel
If the Pause button doesn't apply to cutscenes. Have to go to the bathroom or answer the phone? Hope you don't miss the NPC giving you the secret combination to defuse that ticking bomb....
# Bribing Your Way To Victory
Oh, you only had the money required to buy the core rulebook? Sucks for you that the specific material for monster's weakness is in this obscure splatbook. Guess you'll have to risk a Total Party Kill to complete this module, poor person.

these two can really piss me off sometimes

for some reason i always have to take a shit or huge ass piss whenever a escort or timed bomb mission pops up, and the game starts because i couldn't pause and therefore i end up dying and missing that whole scene, and once i get back it assumes i saw and memorized that information so i get to later on in the level and have no fucking clue what to do >=[

and there are some fucking enemies..that do shit so illogical and complete opposite of what common sense would dictate, and you spend hours fighting them over and over until you finally find it in a guide online that turning your back to them and farting kills them in one hit... (not real, just saying how stupid sometimes it is)

It's got its own TV Tropes entry.

So what are the Fake Difficulty game elements that annoy you most?

Mine are snipers. While there are some games that do a pretty good job of letting you know where they are (thumbs up to the Half-Life series for the laser sighting, which at least gives you a chance the first time through), a lot of them just come down to trial-and-error. Raven Software is probably the company that has annoyed me the most with it, transforming the other-wise piss-easy Return To Wolfenstein & Jedi Knight 2 into Quick Save Whoring during the Sniper areas.
But shouldn't spamming Force Protection help against the snipers?

...but I mean any game where one little mess up can cost you 50% of your health and(for me) slight panic, it's extremely frustrating. Games with a flow, should keep a flow, and not ruin it by taking overly long to recover.
Hmm.. Jedi Academy during lightsaber fights and Bushido Blade.
You do one little error and you're dead. Except it's fine in Bushido Blade though, can be frustrating in Jedi Academy.
this. the game was so much fun up until the point i got to those dual wielding mandalorian looking faggots in which sometimes they would kill me in one hit and sometimes it would take literally 5-6 minutes and even then i wouldn't kill them sometimes, i have no idea how the damage or hit counter worked for that but those battles were always a random toss up even when i went in with both power ups against them.

Recently, my brother and I ran headfirst into this when playing BLOPS, the game decided to auto-save us when we were at low health and having to reload, all while standing 4 feet from a guy with a SPAS trained on our head. We had to restart the mission just to avoid the insta-death every time the game loaded.

In Starcraft 2, the Insane bots get 1000+ APM, as well as increased resource collection rates.
I think their build times are also reduced, although I'm probably wrong on this.
