In my opinion, Scrounger is the most useful perk in the game. Until I got it I was always running low on ammo and having to use guns I didn't really want to. Now I'm level 18, and I have at least 300 rounds for all of my guns.

Also, the most useful perk to have in combat is the commando perk. If your energy weapon skill is high enough and you have a plasma rifle plus the commando perk, you can 95% headshot enemies from anywhere. Almost makes the game too easy.

I would recommend heavily against the bloody mess perk ever being taken.
Its one that I liked in Fallout 1 and 2, but it doesnt work well in fallout 3.
I got it and had to reload my game to before it to continue playing. In the orriginal games it was a fun one to take, but in 3 it causes enemies to explode violently from nearly anything - not only is it not realistic at all but it becomes a huge annoyance to try and loot.
Right after getting it I had 2 raiders and the guy with the sniper rifle in the mine filled town explode into giblets from a hunting rifle round. Looked cool, but unrealistic. Then I wanted to loot them and I had to find WHICH giblet was the one that counted for looting - the torso that went flying 30 feet away into some bushes. I quickly found that keeping that perk would just become supremely annoying as I continued the game - and I get enough exploding heads and severed arms from VATS without it.

Only? I have thousands for most guns. Only about 500~600ish for energy weapon munitions. My 5.56, .32 and 10mm counts were into the thousands when I finished the game. My minigun ammo was way high as was my rocket and mininuke. And I didn't have scrounger on. The only thing that ever got a bit low was my .308 and .44 because I loved the rifle and Lincoln's Repeater. And that was without scrounger. Then again, I did buy a lot of stuff. My evil character was based out of Tenpenny Tower and I frequently bought and sold with Gustavo. My good character is having a bit more trouble but not too much trouble. Having a ton of missiles makes things a bit easier.

Also, I don't find much more extra on people. Mostly in containers.

Edit: Also, I prefer the laser rifle. I had my AP boosted a bit and I would get about six shots from a single entering of VATS.

Get Solar Powered. Most people will complain about it only being usable outside from 7 to 7 but think about it like this. The buff it gives is nice, but the main thing I use it for is healing. I hate playing the game at night because I cant see crap anyways so I end up waiting for like 12 hours to make it day again ever few minutes. So the only time I cant use this perk is inside. But if you let yourself ALWAYS heal to full health just by being outside, you never have to go looking for beds and you can save all of your stim packs for emergency mid-fight healing. That saves you from ever having to worry about stim packs ever again provided you make sure to open medical kits you find.

Oh yes, because the Fallout series is known for it's realism. You're fighting gigantic mscorpians and super crabs, blowing up super mutants with a mini nuclear catapult and blasting dudes in power armor with pew pew pew guns. Realism isn't a very valid complaint. And yeah, sometimes blasting dudes can make looting a hassle, but pretty much every meaty chunk can be looted.

Next go through on the game I am going play addicted to every drug I can and not get the good doctors to relieve it. (I'm going to do high barter so I can get a lot of drugs cheaper)

It's not totally unrealistic. Shoot someone with the right kind of bullet and it will happen. Never mind that you're in the future. Obviously all bullets are loaded with body-explosion capability. I didn't use it on my second character though, as it's mainly an entertainment perk and I don't think that 5% is worth it. I'd rather be assisted by Yao Gaui

Honestly, I don't think my Mini-nuke needs the assistance. In damage or exploding people bits.

Any perk that increases my hit rate in VATS and Grim Reaper's Sprint are usually my first choices.

Bloody Mess is fun the first few times but if I can keep it semi-realistic I'd like to. I just can't imagine the ballistic force of a .32 caliber bullet causing all the limbs to splay out like paintballs...

Mysterious Stranger is also occasionally cool, after all having this noir guy come out of nowhere and shoot your enemies in the face with a magnum can be satisfying, but Murphy's/Sod's law ensures that he'll only appear when I don't need him.

Big guns doesn't get much support from all the perks which is a shame though once you get Vengeance and the Burnmaster you can easily slaughter anything.

It depends on your character.

A high intelligence character is probably going to have enough skill in medicine to make solar powered irrelevant.
Grim reaper's sprint is going to be irrelevant to anyone who goes the stealthy sniper/ninja route.

As for the best perks in the game, I can't say I'm adverse to Demolitions expert. Nothing says "Unbalanced" like 3 bottlecap mines or 3 nuka gernades killing literally everything short of a Behemoth.

2+ Strength is bad?

Also Grim Reaper is sexcellent if you use the reaper. If you can get in two hits with VATS using it a behemoth will have to worry.

But lacking health makes it fun. :D I go out with nothing with me sometimes and just beat the hell out of anything unlucky enough to cross my path. This makes for hilarious outcomes when I come upon something that I'll be heavily damaged from (sm behemoth, anything with a rocket launcher, landmine I didn't see, etc.) but also has hilarious outcomes because my unarmed skill is only 35.

Yeah, it's a real shame, Vengeance is so deadly I put it in the "hand of god/satan" category long ago. At least "Size Matters" gives +15 BG.
