Clive Barker's Jericho had some great creatures. Some were cool

But some were more disturbing

Dementium for the DS had some cool monsters too

Every enemy in The Suffering. Blade-limbed wall-crawling enemies, enemies with glowing syringes for eyes, one-man firing squads, torsos that drop from the ceiling to strangle you, bloated corpses that open their stomachs to release plague-infested rats upon you, little burning witch-girls...yeah, creepy enemies in that game.

I say the most fucked up creature is something that not only takes pleasure in killing people and things in imaginatively horrible ways, but half the time derivates from this and just rapes them to death instead. For this reason, I nominate Pyramid Head.

Then again, any creature that can mutate from human to monster, then spend its time tracking down biologically related members of its family to lay eggs in their stomachs via mouth rape, I'd say is about as fucked up, so I also vote for the G-Virus monster from Resident Evil 2.

The dudes in the Hollywood sewers in Vampire: The Masquerade Bloodlines. Three skinless torsos stitched together, two of which are upside down and use their arms as legs, kind of moving like a spider that would make Satan himself scream like a little girl if he saw it in his bathtub (of skulls).

Hmm... My vote would probably goes to the final boss in Earthbound. That boss always make me say WTF every time I see it. Mainly because its really hard to explain what it looks like.
And, if memory serves, that "thing" is a human infant. Giygas is the name.

And I agree with this person. Nothing can top Giygas, particularly his second form.

Ohhh boy. Well, the Silent Hill games have already been mentioned, so I'm gonna put my vote in for the Shadow Hearts series. Truly some of the most cracked-out monster designs I have ever seen. A few examples:

-a German Shepherd with a human arm where its face should be; it balances on the hand and attacks by hopping up to you and beating you with the dog
-a giant frog with human faces for eyes and a bloody hand for a tongue
-a human infant with a huge ball of random body parts where its legs should be
-the Rage Demons. Just...the Rage Demons. Yeah, that's supposed to be an umbilical cord sticking out of the front of them, but I DON'T BELIEVE IT. XD

I would also like to nominate the Xeobs from Lands of Lore: The Throne of Chaos. Garishly-colored Lovecraftian things with shapeless bodies, vertical mouths, tentacles, and the ability to spit acid. Gyargh.

Most everything in Call of Cthulhu: Dark Corners of the Earth. Particularly the Shoggoth.
I remember the Shoggoth from reading At the Mountains of Madness...it sounds fucked up.

Never got to play that Cthulhu game, though...I am so disappointed in myself.
