It is a 2 way tie for me, between Jak 3 and Jak X. Holy crap, I think I currently have the oldest game on her.
This, I too love Jak 3. Pure awesome.

Probably Oblivion or Fallout 3. Either one I could play for hours without food or rest.

Such a hard choice to be only 1. I'd have to choose 3: Grandia 2, Super Mario 3 or Street Fighter(any of them).

Star Control 2. Explore the galaxy and make new alliances in an attempt to free the home world from the Ur-Quan masters. Great game with good writing and charming new alien races.

Resident Evil 4, I've replayed this game so many times and it never gets old. I love challenging myself on each play through by doing special thing like no healing items for example. Of course the gameplay is my favorite part, over-the-shoulder aiming and a laser sight, it is awesome.

Legend of Zelda: Wind Waker. Not a fan of the series at all, but Wind Waker was amazing. The only flaws of the game was that it was easy and the Tri-Force hunt near the end got a bit tedious. (It did take to you to some neat locations though) I liked the sailing personally, made the world actually feel big.

If I had to choose, I'd say TF2. But a lot of that has to do with the awesome people I have to play the game with, so after that would be a tie between:

Deus Ex: There isn't a lot I can say that hasn't been said a bajillion times. Play this game, and they had BETTER not screw up Human Revolution

Demon's Souls: I loooooooooove this game. I love the atmosphere, I love the way every weapon feels different, I love the controls, I love that it's challenging yet fair, I LOVE how the multiplayer forces me to stay on my toes, I love the bosses, I love how much of a badass it lets you feel like. There's a few minor gripes I have with it, but the positive FAR outshines the negative.

Right now, probably Xenogears because I just replayed it. You just don't get that much depth in terms of storyline in video games anymore.

Kinda hard to say. My favorite at any one time fluctuates frequently. However, if you go by hours played, number of playthroughs, and so on, it would probably be Mass Effect 2. Great story, deep characters (if you bother to actually talk to them and do their loyalty missions, that is), and fun gameplay.

In second place, as of now, would probably be Vampire The Masquerade: Bloodlines. Well worth the $5 it cost me on Steam. Sure, it's a bit (read: quite) buggy, but the customization in the game is quite deep, and the characters are memorable. Plus, a game that old freaking me the hell out with the atmosphere of one of its levels gets props. Ocean Hotel...*shudder*

Tie between:

Planescape Torment

Hard to choose considering there are so many good games out there, but these two impressed me the most.

For me it's Gears of War. Sure it has some cheesy cutscenes and stock characters in it, but I thinks its awesome and I used to play it through over and over no matter what!

I have played FIFA 11 quite a lot but because a new one releases every year and its nothing special the FIFA series probably comes second on my list.

Dragon Age: Origins
Final Fantasy X
Mass Effect 2
Fallout 3

I really can't choose which one, I love them all so much equally.

For SinglePlayer: I'm gonna have to go with Civ4, but an honorable mention goes out to Fallout 3.
For MultiPlayer: Well, not MP per se... just challenging my home slices (i.e. friends) for ProEvo/Winning11 on whatever is the current console we are playing. It's a long grudgematch between 4 friends that has been going on close to a decade.

Infamous 2. The story and characters struck several emotional chords with me, and the gameplay was great.

Kingdom Hearts 2. Ever...
Bah, KH2 doesn't hold a candle to the first. The whole "press triangle to do a special supermove you just pulled out of your ass that is perfect to beat this boss" thing took away so much for me. I mean really, in KH1 you had to be careful with your resources and pace yourself during the final boss sequence. In KH2, it was all just a test to see how fast you could mash triangle.

Plus they really flattened out the difficulty curve. In KH1, I couldn't beat Sephiroth with level 99 and the ultima weapon. In KH2, I beat him at like level 62. Easily. I literally finished the battle and said "That's it?" In KH1, those extra bosses were intense. In KH2, it was as though they didn't want to challenge the player too much, lest they miss a few of their pretty cutscenes.

So I guess it should be obvious by now, my pick for this is KH1. Though Birth By Sleep is a very close second. That is, by far, the most satisfying KH game I have played since the original.
