I know exactly which mission you are talking about and I agree- that one is a *****. But like everyone is saying, you just have to keep practicing. What I found worked for me in the end is after you pickpocket the medic (you will need to use a smokebomb) start running and call your horse. Otherwise you might lose what little time you have after some useless minstrel trips you up.

Don't bother free-running. I just took a horse through the streets to get to the guy.

Try jumping. It's surprisingly useful when trying to travel quickly street level.

Someone swinging from behind? Jump out of reach.
Dudes blocking your way? Jump through the gap between them.
