In fallout 3 I was fighting supermutants on the upper level of a bombed out building. I did an uppercut in VATS to one of them with a deathclaw gauntlet. His head exploded and he flew out of an open window. Classic.

In cp_steel (TF2) I was a scout with force-a-nature, a enemy scout was on the final point (before C was capped), trying to out manuver rockets and bullets, he jumped away to pit, and jamp back using his double jump, but a second before he landed back, I shot him, pushing him very slighty backwards, but enough for him to miss the jump.

I could imagine him scream "NOOOOOOOOOOOOOOOOOOOOO!" at his screen as he fell down.

Mine was a long time ago in GTA: San Andrea's. Me and my mate were dicking about in co-op mode with a few cheats on and flying cars. We were flying and ambulance around the city when randomly my mate goes "F*** youuuuu" and jumps out the ambulance. By some mirical my friend managed to survive the fall of a few miles and starts gonig "WTF i lived I AM GOD".This was soon followed by the sound of ambulance sirens as i crashed down of top of him and kill him.

At the beginning of Fallout 3, when Butch asked me to save his mother. Since I was going for good karma, I went save her. I walked in, went into vats, and chose a radroach and fired. The Bullet flew passed the roach, straight into her head, giving me a nice light shower off head chunks.

Was playing GTA Vice City i believe. I was in a tank, just minding my own business blowing random pedestrians away. I had 4 or 5 stars and the FBI or SWAT or whatever had the SUV's chasing me down. One of them started coming at me full speed, head on. He ran into the front of me and flew up right in front of my cannon. I fired, that sucker flew.
