Something I've always wanted to see is an RTS/FPS. One player on each team is the RTS dude, in charge of everything that traditionally goes on in an RTS. The other 4 or 5 people on the team are like hero units, playing in first person down on the field.
A game where you throw hats at people, and then they go, "Oh, why thank you, good sir. I'll be on my way now, but I shan't forget this act of kindliness, good day sir.", and they put the hat on, and a bunch of those things from Star Trek II: The Wrath of Khan come out of the hat and go into his brain and eat it or something. Or a kamikazi flight simulator. That would be sweet.

set in modern time a man worships a medieval demi god who bestowes power of transformation upon him but one day he stops changing halfway between man and gorrilla and hes mistaken for the missing link. but by the wrong people. these people capture him and keep him captive too try and prove if hes the real missing link as the key too killing god. they worship the same god as him. one day he breaks free steals a butterknife and kills them. as a punishment for his crimes he is cast away by his god, and the god is constantly sending monsters at him. he needs too fight too survive. drawing closer too the abyss with every monster he kills. he seeks out his family for help. but when he finds them a wraith busts in and kills them. as another punishment from his god he is forced too be haunted by his former familys ghosts. he will not stop until his family is at rest he must find the god whom he despises...the one those men were trying too kill. whom will he serve and whom will he kill, one, two, both, none. with the ever haunting ghost of his family. in the end there will be justice and a loving family waiting for him.
How about true zombie survival? Regular zombies, limits on ammo/supplies. Oh yeah, shit loads of zombies.

How about this? A game so predictable it'll bore your brains out... yes I do think they're clinically insane for trying to pull something like that.
How abouy a game where you kill people for no reason and that is really gory, oh wait Manhunt has already been invented, the only way to make that worse would be to be able to kill children. Or do worse things to children, I could see the Manhunt creators doing that.
A Clock Tower like survival adventure game set during the apocalypse.

How about a simcity-like survival game? you manage supplies and ammo and where guns are, and you have to last as long as possible before the zombie gerbils take over the city, or godzillas, or giant ants, etc. etc.
Call it Onslaught.
OR: an RPG similar to Deus Ex, but with psychic attacks and powers instead of augmentations, and other weapons, ranging from melee weapons to early firearms. Also, set it in ancient Rome.
