A first person shooter set in a small country town, like a northern new york town in the dead of winter, cows farms a town the forms at an intersection, the lay of the controls would be similiar to far cry 2 as you could enter any vehicles, (Fourwheeler, pickup truck, average cars.) The story would play out as you're a seventeen year old drug addict who does the works, heroin lsd coke, and the game will show this in graphic realism, Make it show how much of a crapheap this kid is mentally and physically and the violent acts he commits for his dealer who runs a junkyard which holds bodies and drugs in the crushed cars.
Also show his human side, how his girlfriend loves him and after a night of conversation and romance she tries to convince you to get off drugs, you can still do them after the encounter but the dialogue will change although the outcome will remain the same, your dealer either pissed because you stopped or concerned you might kills your girlfriend and you are burned beyond recognition and thought to have died in the ensuing carnage.
Shit hits the fan as cops, your girlfriends family, dealers, and general populace come out of the woodwork at the atrocity, at war with each other the entire place breaks into factions and subfactions that you have to avoid or help.
To explain why they don't notice you're extra crispy is that your character covers his face with a hood and balaclava which would make sense because of the bitter winter weather along with gloves.
Things you have to deal with is your drug addiction, you can use them if you want or not, there is plus or minuses to either side, increased abilities, better accuracy, sluggish movement, or increased. All of according to what you're doing, or not doing.
Guns would be typical for a country town, Cops and dealers would have the best and killing cops would send hell on you and dealers would make the other dealers aware and they won't want to do business unless you can prove you wont kill them too.
You can shoot all one handed weapons while driving with decreased accuracy and think, gunfights on quads through an untouched powderfilled feild would be cool, blood splattering on the snow and stuff.
The ending would be based on who you worked for, either being good bad or neutral, bad would be if you worked for the dealers, pushing drugs and ruining peoples lives. Neutral would be if you worked for the general populace and good if for your girlfriends family. In all three endings you would go to the man that killed your girlfriend and in the fight you'd fight through a druglab attacking each other with syringes filled with heroin coke would be flying through the air and the pot would catch on fire with meth fumes floating in the air, that would be epic and the more time that passes the more screwed up you'd get with hallucinations and such.
The endings would be with you either dying of overdose being arrested or leaving town satisfied with avenging your love.
