Also, money from dead animals. I understand no one wants to have to go out of the way to get money, but killing, picking up, and then selling the creature takes about 3 seconds extra at a shop, and is much better because you can choose to save it until you need it.

In Legend of Zelda: Ocarina of Time, Link saves the world, then is sent back in time to before the world is saved, but the world remains saved.

Any game with a lightsaber. Not because lightsabers are impossible (you can do something similar with plasma with current technology.) Rather, I can now cut through anything, walls and doors mean nothing to me. That's only the beginning of the problems. If Miyamoto had access to a lightsaber the book of Five Rings would have been a whole different read.

the rocket from brute force.. my freaking god! firstly its homing from a semi-automatic launcher. if thats not defying physics then the next part will. every single frikken rocket splits in 2 every second. so per say you happen to be in one end of the map and you pick of the "kill everything!" (as i like to call it) it does what the name says. by the time the enemy even sees the wall of flying rockets they know they are dead. one time my friend and i counted 60 odd rockets flying towards us before our meaty chunks were disintagrated. i mean serously a rocket that splits exponentialy is plain out amazing.

looting animals for money. Also anything that survives after being shot in the head...by a mini nuke

Or on the flipside characters who never stop eating. Just look at Pac-Man. He spends all his time eating pellets 1/4 of his size with the odd piece of fruit or ghost that are as big as he is. And yet he never gets any bigger.

Headshots that don't instantly kill enemies.

Enemies that can take an entire clip of ammo to any part of their body for no reason. If there's a plot explanation (like with the Ganados) for it, fine, but usually it's done for its own sake.

Characters never eat, almost never sleep, and rarely drink liquids.

Hit points.

Any game that doesn't have region-specific damage.

Not being able to climb a chest-high wall, not being able to bust open a door (despite having all kinds of weapons at your disposal) and pretty much ANY game mechanic from Resident Evil (RE1, RE2 or RE0, specifically).

I guess that means that you never played a Sims title before. It's actually pretty obnoxious to have to keep taking potty breaks every few minutes.
Still, it is pretty infrequent in games in general.
I'll go with characters with no endurance meter or, in the case of fighting games, no ki limits. Not only can Ryu and Ken fight all night and day if noone scores a direct hit but they can toss Hadokens until the Sun burns out!
Worse is games with breeding or evolutionary elements but have no sexual selection. EA was wrong to ever have considered making Spore without risking the dread AO rating even if it left it a digital download only title.
