The Shalebridge Cradle in Thief 3, if you've played it no more needs to be said.
I think probably the weirdest one for me was either in Oblivion or Morrowind when I entered this dungeon/sewer like place.
I went really far in and got lost at this point I think the game glitched and the lighting went really really dim, and I had to creep back to the entrance avoiding traps and enemies in near total darkness.

When I first played Morrowind, as someone quite a lot younger, the first caves I wandered into on the way to Balmora full of undead things which royally beat my arse had me a little unsettled.

Ocarina of Time. The fucking Forest Temple. It's like Silent Hill 1, in that the limited graphics make it even scarier some how.

The Outcasts need a Pipboy or similar device to operate the Anchorage simulator and try to use the pipboy the Gary has by removing it. Unfortunately for him he doesn't survive the process.

I can see how the gary vault could be scary but I found in fairly funny because it was a reference to the scene in "Being John Malkovich" where John Malkovich is sent into his own head and sees everyone as him and the only word they can say is "Malkovich".

That one demon door in fable II freaked me out though, the one that turns from a warm winter cabin into a dungeon and makes a loud screech when it does. SCARY.

Arx Fatalis - Old, not really known game but it has probably scariest tomb ever while rest of the game can be pretty cheezy.

And don't even get me started about the Deathclaw Sanctuary.
This also. I am also scared wandering around Point Lookout for some reason.
When I was walking the way up points in the Pitt looking for steel ingots, I was terrified specially when you get way up top and you feel like your just suspended in the middle of nowhere.

I knew that the Vampire The Masquerade game was meant as more of a vampire-themed RPG than a horror game, but was surprised at some really creepy parts, especially that "The Shining" inspired hotel level.

The Eels from the second Gex game scared the fuck out of me when I was a kid to the point where I ripped my controller out of the system running for the door

The Library level on Halo 1, I think it was mainly when those giant doors sealed you in the sections and the flood would swarm in.

As a kid Super Mario Bros 2 bosses were pretty creepy and also in Super Mario World the final Bowser boss in the scary-as-hell clown bucket thing...for years that was the ONLY mission I hadn't finished.

Sometimes on Fallout 3 it wasn't the areas or enemies it was the logs you would find that had the scary stuff in, you would read them and be like "Right...I'm outta here this is messed up"

I'm going to feel stupid for reviving this thread,maybe not if everyone joins in.

I have a huge list.I dunno if the first one counts,but it's caused by a game.
Here we go...:

  • PS2 RSOD,just couldn't handle the music or the darkness!!!This still scares me at night!
    Super Mario 64 after cartridge tilting,I just never got used to Mario being in the ground like that.Or his glitchy "Ya!" when he jumps.
    LOZ:OOT,if you play this game a lot,it's common to see zombies raping a blonde young adult/teenager.Just nighttime,raining,ghosts scared me shitless.
    Donkey Kong 64 game over screen,some fatass reptiles like to kill monkeys with nuclear weapons and still fit in that freaky evil laugh.

As a child, whenever we got to the blue whale in Ecco the Dolphin I'd have to leave the room cos it scared me so much.

More recently, Ooccoo and son from Twilight Princess were possibly the most unintentionally terrifying things I've ever seen...
