The giant spider cavern in Temple of Xian in Tomb Raider II. For ages I refused to do that segment and would always level skip when I came to it. Weirdly, The Temple of Xian is my favourite level in that game now.

I don't even know why it scared me so much, I don't really mind spiders that much.

Metroid Fusion. After the SA-X reveal and the dramatic music, I shit myself. Also, the water level in Super Mario 64 creeped me out. Oh, and Orcarina of Time, just because of the ReDeads. I still call them Hug Me Daddys, though.

Morrowind... I didn't know that there were zombies in the game so when I walked in to a fort and saw a humanoid shape walking to me I didn't expect something wierd. It were incredibly dark in the fortress but that was pretty usual for Morrowind.But then, the thing started running as i got closer. Just as I lighted my torch I saw it.
I've never been as scared as then, I just ran out and never came back. It's called Corprus Stalker and is scary as fuck.

Btw, sorry for my bad English.

EDIT: And everything in Shadow of the Colossus...It was cool but scary...

Ken's Labyrinth scared me because when my family got a new computer we had a sound card and when I replayed it with the better sound it frightened me (the last level of Jazz Jackrabbit was the same).

Not so much the "Gary Vault" for me, but that vault with the violin had a terrifying backstory, if you looked for it. Freaked me out something fierce.
-Deathclaw Sanctuary, The Scrap yard in The Pitt (something very 'Silent Hill-esqe' about it),

-my first experience with a yao-gui (you know this, running across a dark night time wasteland, suddenly BAM a bear is raping your eyes),
-first Deathclaw experience (also my first Deathclaw flying glitch experience scared me shitless, thought it was doing some pokemon style fly attack or something)
-The Orbital strike in Broken Steel (I was standing right on the edge of it, the first time.)
-First time i ever fought a feral ghoul reaver, ("WHY WONT YOU DIE!?!?!*fires blackhawk into face*)
-First Albino Radscorpion experience (had me ligitimately scared).

probably some other ones too, but I cant remember, probably suppressed memories.

outside of Fallout though? hmm

Fatal Frame made me nearly piss myself once, but thats actually meant to be scary.

Almost every time it was night in Oblivion I was scared. Just so many random noises (and the fact that I couldn't see) freaked me out. The oblivion gates did a bit too at first.

Also, I thought that the castle bit and the last world in Kingdom Hearts were scary as a kid. Randomly floating objects and Escher-esque settings creep me.

Portal scared me at some points. The ratman den particularly. The music made me want to run away and hide. Also the abandoned house in tranquility lane. I knew i wasn't going to be attacked, since i had no weapons, but the general monochrome feel of the place got to me.

Dunwich building tried too hard.

Vault 87 scared me, mostly because the Super Mutants could quite easily kill me. And it was so dark and abandonned and stuff. The Gary vault wasn't so much scary it was just strange.
Fallout 3 generally is kinda eery, especially on low levels, because you're so fragile without big armor and weapons.

Fallout 3, I just killed a deathclaw, I turn around and there is about 4 yao guis, one of which was about 2 inches from me (the screen was filled with its face)

Answering the whole Fallout thing, I really was only afraid of three things:
1) Ghoul Reavers, at least before I found that if you knee cap them they will be far less harmful.
2) I thought I heard a hillbilly say "Boy You have a Purdy Mouth!" The first time I fought super mutants while only equipped with one missle and five rounds of Rifle ammo
3) Rad Roaches. Mainly because I live in Houston where they grow large enough to carry cattle away and also fly.

Anyway, some other games that scared me. I get very nervous and scared playing Terrorist Hunt in Rainbow Six Vegas since that game could be very unforgiving at times and I will get so immersed. I also have been freaked out on Jet Force Gemini, specifically on Tawfret since the music is very creepy and the zombiefied giant ant soldiers look so creepy and move so unnaturally and are very tough to kill. A rather embarrassing one to admit was in Ghost Recon Advanced Warfighter 2, when your interface gets screwed up and you're on your own, it just was rather jarring.

Edit: Actually Four things in Fallout scared me:
4)When you enter that one vault where your screen turns bluish purple, since before an Xbox died on me once the screen turned pink.
