Conquers Bad Fur Day had a very depressing ending but I also found it a bit funny, but probably more sad.

I also found the OoT ending sad because it was one of greatest games I have ever played and now it's all over. :(

accidentally kills another primary character
Also, Penumbra has some really sad moments, including one very similar to the one I just mentioned. Except what happens this time is OUTSIDE of a cutscene. The game has a lot of demented tricks up its sleeve.
I'm pretty sure you don't accidentally kill someone in FE at least not in the remake of the first one. The game says you have to send someone off to their death so Marth can get away. Unless you mean not noticeing a crucial enemy and they accidentaly die from your own Epic Miss. I do that alot near the end.
First GBA Fire Emblem, which is Fire Emblem: Rekka no Ken (techinically, the second GBA game, but the first with a non-Japanese exclusive release) is what Katana said. And, yes, that is pretty damn sad.

As well as that, I'd say Shadow of the Colossus, Kingdom Hearts and both Metal Gear Solid 3 and 4.

Uh...Marth?
If I'm correct, the first American GBA Fire Emblem involved Eliwood, Hector, and Lyndis.
Well... you said first Fire Emblem. Not first American, so I thought...and with the remake that came out recently...and the translated emulators...I just don't understand anymore...

dom killing maria in gears two.
I always wondered why he used a motherfucking handcannon to kill her while looking in her eyes. HE WOULD SEE HER HEAD FUCKING EXPLODE!
I lol'd when that happened.

For me, it's Lost Odyssey when...
Kaim's daughter is dying, they're crying and her children are telling her not to leave them... Just sad. Very, very sad.

Metal Gear Solid 3. The truth hurt. He cried, and I almost did too.

i guess the part with scarecrow in the beginning of batman arkham asylum but it wasn't really sad just traumatizing like when batman opens two of the body bags and finds his dead parents corpses talking to him begging him to get revenge

The whole ending of Call of Duty MW was pretty sad for me.

Was alright though, that random epilogue mission cheered me up :)

Destroying the Companion Cube

I thought the part of Dead Space where:

Issac gets shown Nicole killing herself

...was pretty sad.

The death of Aerith comes to mind, as well as the erasure of certain TWEWY characters and the death of Roland's dad in Luminous Arc 2 was pretty freaking tear-jerking.

(Maybe it's not that bad when you're older than seven, which I was back then)

And of course the end of FF VII
