System Shock 2. Couldn't play that game with the volume turned up. I barely looked at my monitor most of the time as well. I don't know how I got through that game; it was scary as hell.
When I played Abe's Odyssey as a young lad, the first encounter with those dancing beak-thingies gave me nightmares. The image of them dancing upon poor Abe's corpse flooded my dreams. The prior opponents in the game and the gore had already freaked me out a bit, but they really brought the game to another level of intensity.
Dead Space made me uneasy, probably more than any other game I've ever played. Especially that part where the rock smashes through the flight deck.. with those MASSIVE windows.. omg why were they so big. I was constantly worried the ship would crash or I'd be eaten.
System Shock 2 Stopped Being scary after a bit of awesomeness: 4 fucking Hybrids Were behind a door and burst through scaring the shit out of me. I kicked the SHIT out of them, took their stuff. Really, Its kind of like doom, only an RPG. and not for the cell phone.
Oh look, another opportunity for me to mention Amnesia: The Dark Descent.
So yeah... Amnesia. Bricks were shat in short order.
Especially when those bastards with the metal legs started showing up. It doesn't help that they make a terrifying noise when they spot you. A sort of half-roar-half-shriek.
Fallout 3 i was in one of the vaults it was infested with bloatflys i killed some and as i was walking to my objective i realised i had gone the wrong way i turn around 2 bloatflys right up in front of me scary as any thing.
also during Those!! quest when u walk in the tunnels i had a glitch happen that when ever i killed one the the nest guardians the ant queen would magically teleport in front of me i never did that qst again and
