Mount and Blade (and it's expansion) are insanely replayable, there's many different ways to play it. Still, it's very, very difficult to actually beat the game, most people won't (I haven't) but it's just so much fun, and with the ability to bring the same character between different playthroughs with the same level, stats and money, it makes it much easier to get going the second time through..

Ever since playing it in 1994 I find that once a year I get the urge to play this game:

Betrayal At Krondor

other notable mentions:

Diablo 2

Fallout 1 (I have played 3 a lot but consider it too recent to put on this list)

Baldur's Gate 2

Super Mario World

Super Castevania 4

Castlevania: Symphony of the Night. I don't even need to think about what I'm doing anymore. The whole game is pretty much muscle memory at this point.

Dungeon Keeper 2: Mistresses in torture chambers. That is all.

Harvest Moon 64: I don't even know why. I just love starting with nothing and earning money. Then getting to know the people in the town and helping them out while courting Elly (yes, always Elly) and eventually getting married. I always wish there were something more after that. I guess that's why I keep coming back to it: I feel like there must be something more I'm missing.

Kotor 1 and 2, combined I think I've played them around 15 or so times.

Dragon Age: Origins. Not entirely sure how many times I completed it, maybe 4 or 5 times. Steam says I've played it 156 hours, so that seems about right.

Loderunner 2. I've lost count of how many times I've completed it and how many hours I've played it. The replayability value of it is just immense. Completed the levels in different manners and trying to beat the already set times or your own times are fun. Or imposing additional restrictions to make for an even greater challenge, like doing a marathon run(no saving), while still trying to do it as fast as possible. Good times.

Also, I'll have to mention World of Warcraft. I've logged plenty of days, not entirely sure how many, but I'm pretty sure it account to more than a couple of months.

Also, I feel Baldur's Gate 2 and Planescape: Torment being added to this list in the coming years. I've only recetly discovered both of them and currently on my second playthrough on both of them as well.

Soo, I recently finished Silent Hill, the first one on the PSX, for the 8th time!!!
Nice! That's one of my favourites as well, I complete a Silent Hill game every Halloween I've noticed, completed the first one about 3-4 times now. If you have L4D you should really check out the L4D campaign.

The game I've completed most is probably DKC2, all 3 on them on the SNES are awesome but 2 is my favourite, completed it many times since the 90's. I love everything about that game.

Mass Effect, it just feels so immersive. Also I need different imports for ME2 and ME3.
Halo series. I love the story, atmosphere, multiplayer, characters, etc.
Dragon Age 1&2. I love the characters and story.
Mirror's Edge. Free running is awesome.
Bayonetta. It's the most pure fun I have ever had.
Fallout 3 & New Vegas. I'm not sure why I keep replaying them. I don't think they are great, but I really enjoy them.

Dark Messiah of Might and Magic
First Person swordfighting and spellcasting! Awesome! With interesting multiplayer too.

Metro 2033
Great immersion and atmosphere with pretty good shooting mechanics. Really tense when your gasmask is cracked and ready to break when precious air is so far away.

Morrowind and Oblivion
It's Elder Scrolls, need another reason?

Fire Emblem 7 on the GBA over and over again. Its such an awsome game; nothing other than the overarching story is the same on each new playthrough.

And yeah, RE4

I've played Oblivion for 4,000 hours, so that. I really dig the enemy scaling. Allows nothing to be over-powered or under-powered, so anywhere I go at any time offers consistent difficulty
I have a phobia of vampires and caves in that game because every cave has a vampire in it and every vampire gives you a chance of getting the vampire desease which if youu don't get cured in three days you have to do a buggy as hell quest to cure it and also I am scared of the dark in that game so I have my brightness turned up because of these phobias I can not play the game that much but I still have played it alot
OT: Oblivion 400+ hours

Diablo 2. I've played this game for nearly 10 years now, have it installed on all my pc/laptops and play it all the time till bored, then take a break of several weeks/months then play again :). With the EasternSun mode, it has so much variety that you cant let go of it. One of the best games ever.

Fallout 2 play it once a year every year; for the past 11 years. It's the sole reason why I got into games as a young teen. Before Fallout 2 I only saw games as something only boys and kids play.
