Haven't completed too many games on a high difficulty.

Half-Life 2 (I suppose this doesn't count as it was insanely easy on every other difficulty)

I go through every game on a low difficulty then play through on higher or possibly highest difficulties.
So in answer to your question everything. I have played everything I own through on the highest difficulty. It would take too log to list everything so I'll just say everything.

None of them have beaten me yet. I've finished them all.

Touhou EoSD on Lunatic. (all 3 continues but by god I did it!)
And a slew of others but Touhou is definitely the most memorable.

I suck at hack and slash games like God of War and Bayonetta, so I played them on normal. Everything else, I don't go below Hard. I did this in Vanquish not realizing how unforgiving that game is. It kicked my ass pretty hard, but the challenge was fun. I lost count how many times I had to retry the final boss(es).

One that really pissed me off was Veteran on MW2. I kinda suck at shooters so I was super pleased with myself when I got to about 1 or 2 missions from the end, when my Playstation decided to crash and not turn on again.
I had to get a new one and I hadn't backed up my hard drive for a few months :( Couldn't be bothered to play it from the beginning again to get that gold trophy for completing the game in Veteran I was aiming for since I was so angry.

I like to play a lot of games on the hardest difficulty. My favorites in paticular are the Fallouts and Oblivion. The hardest part in Oblivion was getting out of the starting dungeon. Even talking about Oblivion is getting me more hyped for Skyrim!

I don't keep track. Most games I like I'll beat on the hardest setting. Really easy games I'll put on the hardest setting for a challenge. I'll prolly end up playing through skyrim on the highest level second or third run.

generally, i only play on hard if there's an unlock tied to beating it, if its just an achievement, then I'll probably never up the diff

Fun with a little more challenge, but some of them was not fun at certain parts (The stairway before the HinD Battle in MGS1).

I usually start on the second to hardest difficulty. Then - provided I like the game - I'll finish it on the hardest difficulty. So yes, just about every game I've ever owned I finished on the hardest difficulty. Dante Must Die in DMC2 and European Extreme in MGS2 are my most difficult accomplishments by a very, very long way.

I can't think of many, but I do remember besting Gundam Crossfire on Very Hard (an underrated game in my opinion, but I am a whore for Gundam games).
I've also taken Doom II down on Ultra-Violence mode (Nightmare difficulty is BS). Unlike modern shooters, Doom 2 only has 2 enemies that are cheap. The rest of the challenge comes from the hordes and non-regenerating health. It also doesn't save checkpoints automatically. If you forget to save for a few levels, you are going back through them when you inevitably die. Doom II does not fuck around. I use a mod to make it full 3D with vertical aim control (Risen3D is what it is called), so there isn't autoaim like there is in the vanilla version.
