In MGS4 during the Crying Wolf fight while on The Boss Extreme difficulty. I became frustrated waiting after about 45 minutes since my last shot, so I decided to randomly shoot a bullet into the trees with no idea where she was, and hit her just as the machine opened up and killed her with a headshot.

Me and a friend were playing Resident Evil 5 and figured there was no way we could kill one of the parts of the game and said, "Fuck it. MY LIFE FOR AUIR!" (A thing me and my friend say when we decide I don't care if we survive.) We pulled out or knives and just knifed the hell out of everything and somehow we survived.
Oh and in CoD4 we were down 7100 to 7400 in vacant and friend of mine threw a cooked grenade into a room of 4 guys and downed all four of them. All I gotta say was there was so much screaming and cursing you couldn't hear a damn thing in our Xbox Live party

An F-Zero game where on the last lap of the race I was down to the last verges of boost, and if i took one more hit, I'd be dead. The inevitable happens and I'm sent flying off the track, over the finsh line. Bloody lucky that the game decided to sent me in the direction of the finshline, at the right velocity.

I was fighting Raam on Gears of War 1 on insane with a friend.
We pretty much figured uot that i he got even remotely close, we died.
Just as he makes his way around the cover we were behind, I fire my sniper rifle, get a no-scope headshot, and win the game.
Both me and my friend said,"never again" after this.

So I went in to Arlington in Fallout 3 via metro only to find myself being buttf*cked by five super mutants. So here's me, without any major explosives save bottlecap mines and a stinking dog companion that's apparantly trapped on the other side of the fence or something. I won. =D
My newest is on Bioshock 2 .There is a part where you must hold back a certain number of enemies for someone to escape and i had very low health(playing on hard),6 guys start running towards me and i panicked and started shooting electric bolts.One of the bolts hit a well placed canister and 4 of them died.The other two got electrocuted by water.

Halo 3 Legendary, me and 3 friends attempting the vidmaster on the last level, no checkpoint for about 5 minutes, all of a sudden like four flood-brutes charge at me, so basically if one didn't kill me the other three would most definately enjoy it. At this point Sgt Johnson decides this'd be an excellent time to actually target some enemies who were attacking us rather than nothing, he kills three of them and i somehow assassinate the other one.
Twas amazing for me, unfortunately for us one of my team mates suicided with a badly thrown sticky about five seconds later.

Playing Saints Row 2 I was on a mission where the rival gang ( Forget wich ) had rocket launchers, I was speeding up in my car, they fired and I managed to dive from the car as itgot hit then get my uzi's put and fill the guy with leds.
Also once in Oblivon I was on like a fraction of my health being chased by 5 Maurders in a ruins, I run around a corner, down some stairs in a leap and land, then the maurders come charing down, onto a spike trap and floor falls and they all die, I was like 'Holy crap that was a stroke of luck!'

I am not very good at MW2. As such, I was currently on a death streak. That death streak was final stand. Now, the score between the two teams was exactly tied, next kill would win. Me and another player spotted each other and exchanged bullets. I promptly dove behind my corner, as I was hurt. He came around a shot the necessary amount of bullets into me to kill me. Seeing my body fall, he turned away, believing the game was now over. I killed him in Final Stand and we won.
GTA 4 has plenty of examples of me missing a vital chance to take out a target (Nico was too busy having a cutscene when they were right in front of him) and then a shitty driver plows through them.
My first time fighting the last Colossus in Shadow of the Colossus...I didn't know that you're supposed to stab his right hand, then shoot his shoulder, then climb onto his shoulder when he brings his hand over there. Instead, I just took a mad flying leap of faith from his hand out into space, somehow managed to grab hold of a ledge on his upper chest, climbed up to his head and finished him off. I've never been able to make that jump since. XD

not exactly me surviving but still shear dumb luck, as a sniper on tf2 i had been trying to get this guy for ages but he just kept moving about to much (he didn't know i was there)
he ran back into his base to get health+ammo (it was that map duel_duel_2 where it's just on big circle) and i though "fuck this" and took a potshot in his general direction hit him in the foot and killed him
