RE4, Dante's Inferno, L4D2 & Dead Space 2 are good recommendations. Add Shadows of the Damned to that list and Metro 2033.

Killing Floor had some memorable gore aspects, and it's fun as hell, my second most played game. As did Left 4 Dead.
I don't know if you could find a rom for it (or how you would even play it since it was an old light gun game) but there was an old arcade game from the 80s called Chiller that would probably be rated AO if it came out now.

The Darkness 2 and Space Marine are both cheap right now and should scratch your itch for gore. I won't post any videos because I don't want to spoil that feeling of satisfaction when you see a gory kill for the first time. In D2 you play as the head of a mafia but the real fun is using your two demon arms. Yeah you have those 
. In SM you play as a -gasp- Space Marine trying to stop an Ork invasion which admittedly sounds generic but, trust me, it's fun when you're mowing through waves and waves of space-orks.
Expect a short but fun single-player of brutal death for both!
