Guild Wars 2 was actually spawned from the expansion process for Guild Wars. As they built and refined in Guild Wars, they reached a point where they honestly had to say that they couldn't do what they were envisioning for the game, given the existing code and design.
Rather than chase the easy money that further expansions of Guild Wars would have brought them, they took a rather bold step and announced that they would stop expanding Guild Wars and create Guild Wars 2. Remember, Guild Wars was their first and only game at the time, and had been an unqualified success.
Lots of folks have knocked the manifesto video and other media as hype, but this game has been in development from before they stopped expanding Guild Wars. So, GW 2 has been in development from at least mid 2007, if not earlier. If they release the game in 2011, which it looks like they're on track to do, that's a window of 4+ years development time. That's a long time to go without anything other than game purchases/store purchases as the only revenue.
ArenaNet surprised me with the original Guild Wars, it may be that they'll have lightning strike twice with their next game.
