IWBTG should not be listed here. It's hard because the devs decided to do thinks like having invisible platforms and other such things that you can only get through with trial and error. The same is true of demon souls. It's meant to get rather easy on the second play through.
A game that's really hard is one that you cant figure out the patterns to, memorize the locations that you have to jump to etc, that would make it substancially easier on a second play through.

Rugrats: Time Travelers for the Game Boy Color. That's the single most unfair game I've ever played.
I got it when I was about seven, back when Rugrats was the shit. I still can't beat the first real level. I mean, you're supposed to collect something the game doesn't specify, then go somewhere you have to discover on your own, all within a crazy time limit. Oh, and there's no map. Good luck.
