I've played and beaten alot of games in my day, don't own any current gen consoles as of yet. Here are the ones that I either never beat, or did once out of pure luck:

Alma (first battle) - Ninja Gaiden XBOX
I can beat her, but it's never from skill, pretty much entirely luck. Second form is pretty much a joke though unless you're careless. This game had one of the hardest difficulty curves of anything I've ever played

Chaos - Final Fantasy I NES
On the ports like the GBA one he's super easy. The only way I've ever done it on the original is with an emulator and save states and even then it took 50+ tries, my party was level 29 or 30

Dr. Wily - Mega Man 7
Been gone over already. For the record I am a Mega Man god, can beat most without dying unless I'm careless, but I've only beaten 7 once on account of wily being near impossible.

Metal Sonic - Sonic 2
Anybody who's played him knows why. Changing pattern and no rings so one-hit death.

The whole game - Metroid NES
One of the toughest classic games, if i was even lucky enough to get to a boss still alive.

Aries final form - GOW 1
very difficult battle especially with the health system used. GOW2 bosses were alot more fun but a joke difficulty-wise compared to this guy

2) Spikey Tiger from Secret of Mana. He's got magic and you don't. He can spend over half the fight out of your reach peppering your party with fire and paralysis magic free from harm unless you're good with the bow and he's not targeting you. When he finally DOES get off the damned ledges he will always slam down on you in 'ball' form to damage you and knock you out for a good 5 seconds, again hard to dodge for you even if you forget about charging up and just run, absolutely impossible for your CPU allies. You will typically take 100+ damage divided amongst your party for every 30-40 you inflict on him!
His upgraded recolor 'Blue Spike', while still tough, is nothing compared to this beast
Really? Sure he's shit annoying because of that flame move that immobilises you but I didn't think he was overly hard. Actually none of the mana bosses are much challenge, especially with magic (although some like the slimes are dead annoying because they take so long)

I just remembered two shit hard bosses, the two extra bosses from FFV, Omega and Shinryuu. Omega couldn't be hit with weapons (instant miss) had permanent reflect on, countered every move of yours with one of his own, usually a missile which halved your health AND confused you, had an annoying ray move which dealt 4000-5000 damage to every party member and was generally insane.
Shinryuu was found in a treasure chest in the last place, and opened (before you got a chance to even attack) with a move that dealt 6000-7000 damage to everyone and could use that whenever he wants to. AUUUGHHH! I don't think I ever beat those extra ones

-> Demon Gate - Final Fantasy VII - PS1
On my first play through I found this boss difficult, but on subsequent visits I haven't had any problems. (Side Note: Wasn't there a special tactic or something that killed him easily, throwing an item or casting a curative magic against him?)
I think time magic worked against him such as slow, I can't remember so don't quote me on that
For me it was all about the limit breaks
The basic strategy (for me at least) was to cast Slow on the boss then caste Haste on your party. Then just beat him senseless.
If we're talking about cosmo canyon all you had to do was cast a pheonix down his way you might not hit him the first time but once it does he dies automatically, as with other undead monsters.

First off: The final form of Dracula in Reinhart's story in Castlevania: Legacy of Darkness. First time I tried, he started off with the triple exploding fireball that takes 50% of your health each. Didn't even give me time to use heal packs. After you die, you lose your powerups, and have to start with Part I of III. Goddamnit.

Also, this doesn't count as a boss, but in Bioshock (PS3) Worlds of Hurt Challenge room, trying to beat the 4 Big Daddies with only a wrench and plasmids on normal. Would have been easier if I remembered to grab enrage.

RAAM on Insane. I beat him after a few attempts in co-op with my brother, but it was still a ***** of a fight each time.

Definitely Penance in Final Fantasy 10. 6 some odd arms each with something like 3 million hp and so many "ikillyouyoudead" attacks that it made me sick. Didn't make it any easier that I don't read Japanese...made Omega from FF8 look like Garland.
The arms have 500k HP and Der Richter (or Penance) himself has 12 million HP. He wasn't that hard though. I killed him with Rikku and Wakka just for fun. The fight is just tedious because it takes about 40 minutes.

psycho mantis is pretty hard. well just finding out how to touch him is the hard part, once you call the colonel enough on your codec then he's easy.

In any game he's in: Sephiroth- has to be the most difficult boss to ever deal with.
Resistance 2: The Swarm- I don't know why but that boss always gave me a hard time.
Ninja Gaiden Sigma: the whole damn game- I have never played a game that I coul'nd beat, that was untill I played NGS the whole game was a disaster waiting to happen for me.

I don't know if anyone remembers one of my favorite games: FFTactics

The boss in Riovanes Castle, one who could summon Titan and crush 3/4 of your team in the first 5 turns, ridiculous. You had to get LUCKY to beat that boss battle, you couldn't really even train.
Goddamn. GODDAMN. He took me FOR-freakin'-EVER! Luckily I learned how to cheap-ass kill him.

You could throw Lou from GH3 on here if you want, along with Virgil (3rd fight) from DMC3, Galamoth from Castlevania SotN (unless you have the cheese strategy), Mizar (2nd fight) from Jet Force Gemini... and others. I forget.

"Your top 5 boss battles"
I know I said there was a thread for 'Best Boss Fights'. This is Hardest.

------------------------

On topic, I just remembered the hardest boss fight ever. Playing 'Super Nintendo FC' in Super Soccer on the SNES. You played them after beating 'Nintendo FC' in the Final. They were incredibly impossible. Best result ever was losing 3-2.

Penance - Final Fantasy X.

Technically a sidequest more than a boss, but, to quote Shakespeare, he's a bugger.
