I've nearly busted my controller on three separate occasions:
1) PS2 Controller - Shin Megami Tensei: Nocturne, after getting a beat down from the first Dante fight for the 7th time.
2) Xbox 360 Controller - Mass Effect, trying to fight the evil Ansari lady during the first playthrough (before I figured how to upgrade my stats correctly)
3) PS3 Controller - Killzone 2, that got-dang ATAC

Yes, once. Three Leaf Clover on GTA 4 sent my 360 Controller on a little aerial adventure. It wasn't hard enough to break it or anything, but it certainly was a frustrating mission my first time through.

I did while playing one of the arcade games in Bully. The PS2 controller shattered and that was inconvenient. I can sympathize with the frustration of the poster who got hacked off at the Matriarch Benezia fight in Mass Effect; I would have thrown my controller if I'd had one to replace it.

I have to say that I am a more of a calm person, never broken anything after playing a game.
Have had a friend that I played halo 2 with online, something about a lag that made him miss a headshot. (he gets very easily angry when losing)
He took the controller and threw through a window, landed in the mountain side 50 metres down from his house intact. (amazinginly, lots of rock there.)
But it didn't work.

It was the only controller he had.

To bad for him.

In Grand Theft Auto: San Andreas every racing mission I would fail up to five times and those failed times I would throw my controllers at walls, punch my TV, and yell at my TV. Good Times.

i started playing COD:WaW on vetren and the constant getting blown up by grenades drove me crazy, so i finally had enough i was only meant to chuck my controller on the floor in anger and instead the whole thing came apart.....oops
Same cause, here, but the for me score is as follows:
Controller: 1.
Drywall: 0.

I guess I threw it so hard over my shoulder that it put a small hole in the wall and survived the impact without a scratch (just a little chalky powder on the handle). Or maybe I just need to invest in better drywall.

...stupid Japanese soldiers that know how to cook grenades. Is there really any way to defend against a grenade that goes off in one second and take two seconds to run from? Also, getting flanked by bayoneteers that don't even bother to activate the QTE and just instantly kill you. Rage.

Most recently, street fighter 4, this things match up system is terrible, I'm getting paired up with people leagues ahead of me in the game when it comes to skill level, it's frustrating, I already have to deal with the controls since I only have a normal controller, and I'm pretty sure half the people I'm p[laying are using those special arcade sticks.

It got so frustrating that I kicked over my computer in a fit of semi-rage, since I was sensible enough to kick over the computer that i don't use anymore.

Call Of Duty 5.

Firstly, I get to level 65. Yes, I'm proud. I laugh in my friends' faces, as they do to me.
Then, I log on next day. "Hmm, where'd my ranking go?"

THEY PUT ME BACK ON 0.

I threw my controller onto the floor >:O

I'm quite sick of that game now but I am level 62 on prestige.

Yes, many a controller has met a violent end in my hands as have some keyboards mostly while I was still playing WoW. Dualshock PS2 controllers have been the most durable in my experience, N64 and Gamecube controllers the weakest. For keyboards I recommend the ugly 90s ones that have the old style plugs. Those things are absolute tanks as keyboards go and practically free if you can actually find them.

I used to be horrible with frustration, but lately I've gotten a lot better. Most recent example was a few years ago with Castlevania: Curse of Darkness. I was playing as Trevor and got stuck on the first fight with the main villain (forget his name). After 45 tries (I counted) I decided I was done, and flung the disc into my closet. It only got one nasty scratch, but it was enough to make it unplayable.

Oh man, there was a game...but I believe I ended up smashing my head against my desk until I forgot about it, which apparently worked. All I know is that I broke about 3 controllers trying to play it.

Ultimate Saga too. Not only was the game bullshit, but it was just disappointing. Shame on Square for releasing bad games recently.

When I was a young teen if a game was getting really frustrating I'd beat on my desk. Nothing ever was broken but thinking back I am a little embarrassed I got so worked up over stupid things. It happened more with things like dling a huge file and near the end something would mess up and it wouldn't dl for whatever reason, leaving me going "ahhhhhhhhhh!"

I have gone through several computer mice. One was from clicking so furiously that I caused an internal disconnect. Another couple have had cords/USB attachments broken.

I also managed to drum so furiously trying to keep up on rock band that I broke the red drum pad off the drumset.

star wars galaxys before the freaking combat upgrade [if you played this game enough it will all make sense] I just unlocked jedi after 2 years of struggleing through the professions finally I had done it! The old man came to me and told me of the secret place [cant remember were on Endor someplace I think] I finally made one of the most rare items in the game my double bladed lightsaber with a yellow Crystal and Kryat Dragon pearls [those things are a pain to get] to focus the beam for more damage. I leveled up a few times gradually taking points out of my main classes [bounty hunter/rifleman]. A week later the combat upgrade came out, NOW EVERYONE CAN BE A JEDI/SITH the class was nerfed no-longer the most powerful in the game and I could no-more be a bounty hunting jedi of legend that pissed me off, and I quit the game that week. [I didnt break anything but thats as filled with rage as I have ever been over a game]

I've never actually broken something, come close once or twice but I only ever throw my controller at my couch, or bang my fists against my monitor that needs replacing anyway

Too many games to count. I smash controllers on a pretty regular basis. Here are just three of the many, many games that have caused me to physically damage something or other.

God of War, God of War II, SoTC.
