Wipeout 64 could be... irritating. I didn't quite break my friend's controllers, but smacked 'em a bit.
Well, on COD4, I was playing veteran on the last level, and I became so incredibly angry that I took my pocket knife and gouged my controller with it multiple times... yeah... I still use the controller though. Another incident of controller violence was when I was playing Soul Calibur 2 back in the day, I was using a wireless xbox controller, and the utter cheapness of the enemies in weapon master caused me to smash the controller against the back of my couch, breaking one side of the grip...
While playing Mario and Luigi Super Star Saga(embarrassingly enough),I got soI started biting my controller.
Same thing happened with NES Tennis.
And Halo 3 when my friend got that cheap alien plane.

Every time I made it to the three-million mark on Geometry Wars without dieing, and then proceed to watch helplessly as a cloud of worms (the ones you have to shoot in the head) spawn around me. At that point in the game, its a complete shit storm, and the game will relentlessly pull dick moves like that to keep you in line.

Ninja gaiden not because it was hard or frustrating but because my Xbox would only play it every 5 or so powerons even black wouldn't play, and the menus would take about 15 sec to load and if you moved the joystick before then it would lock up. Or it would lock up in cutscenes, you had to hit A at the right moment (right when it went from black screen to cutscene) to get past certain ones. Thats about the time I started beating on my black beast...
Stupid Ninja Gaiden.... Sure, I opened and fixed the controller, but yeah, poor thing hit a concrete floor. That is why controllers need to be made tough... Hefty tough!
On the Snow Level in Mass Effect (Noveria?) Where you have to fight the Matriach. I couldn't kill her, and i had to rewatch the cutscene over and over...

I did make several good-faith efforts to crush my Playstation controller trying to finish "Dante Must Die" mode in Devil May Cry 3, but for the most part, I'm more inclined to punch myself in the leg. After all, what did the controller ever do to me?
Wow. After reading some of these I don't feel nearly as bad about breaking that towel rack over GTA3. Aside from that and your average controller slamming/chucking I haven't gotten too violent.
Street Fighter IV was the first time I brought rage upon my 360 controller.
Also chucked a Gamecube controller at....I think TimeSplitters 2, but not sure.
And there was also the wiimote during Super Mario Galaxy, but the strap held on and it swung in an arc to bonk my head.
I laughed immediately after at that.

I was playing Afterlife by Avenged Sevenfold on expert, and I was on a hot streak flying through the song and nothing could go wrong. This was probally the hundreth time I had tried it and I had almost completed it. The trouble began after the solo but made the overdrive sensor less senstive go figure.
