The original Deus Ex. With many, many close calls, like falling to have both legs broken at one point. Did not re-load.

Came close with the original Red Faction, but I think I got noticed twice during the stealthy bits.

I remember not dying once on Chibi Robo. It was pretty close on the final boss though.

The only game I can think of at the moment is Solatorobo: Red the Hunter. However the reason why I had never died once was that one of the flaws to that game was it was too easy. Ok granted I did had the upgrades for more hp and attacks etc but that was out of normal gameplay which the game itself should of turn harder from progression in the game.

Gears of War 3 on hardcore, I did go down but not out on a few points that I pushed too hard, but no full deaths. The following insanity however is killing me quite a bit.

Didn't die once in Wind Waker. Pretty easy feat considering every enemy drops like 4 hearts. The closest I got to death was during a boss fight, where I had to use one of my two bottled fairies.

I played through Undead Nightmare my first time and died once but I don't count it because it was a cheap bullshit death! So yup! I sure did!

Devil May Cry 3 and Star Ocean: Till The End Of Time. Devil May Cry 3 was played on the easiest difficulty which ended up being far too easy. It was still a lot of fun but I probably should've played it on the normal difficulty. I didn't die in Star Ocean: Till The End Of Time because I made sure to train constantly throughout the game, even then it was still a challenge.
OH SHIT! I was getting ready to throw myself at your feet as if you were the Holy Grail of gamers.

Hehehe Right now I'm doing a Let's Play on hard and have gotten up to mission 14 so far and only died once.......F*CK YOU LEVIATHAN!!

Nearly beat Shadows of the Damned on hardest difficulty, but got killed during the Big Boner stage. If I had been on Normal, I would have done it.

Still, pretty impressive, right?

Yes.
Twilight Princess (wii) I didn't die once.

I kind of liked that I could just progress through. Oddly, despite not dying, I didn't really consider the game absurdly easy. It wasn't difficult, but I felt it somewhat challenging.

Every shooter I've played.
Portal, Zelda, Pokemon (well you don't die, but I count that if all your Pokemon faint, you lose), PoP 2008 :trollface.jpg:...... Many other games.

I just don't rush and most games can be beaten really easy.

There's two games i can think of off the top of my head

Fable 3 (in fact there's an achievement for it)
and Bioshock (though admittedly i didn't know what to expect so i choose easy difficulty which probably helped a lot)

Two that I can think of, Starfox Adventures and Zelda: Twilight Princess.

Starfox Adventures was just a poorly executed game in which the enemies, literally, queued up so that you could kill a roomful of them one at a time, it turned the whole game into a joke.

Twilight Princess, I'm not so sure, I enjoyed the hell out of the game, but it just wasn't nearly ass challenging as any of the other Zelda games. Might be that it was nerfed a bit for the Wii motion controls so people could get used to them seeing as it was a launch title but, I dunno.

Silent Hill 3, the enemies took too many bullets to kill, so I ran past everything, after that decision, I barely had to use an health kits, good thing really, the final boss really drained my health fast, but almost an entire games worth of med kits saved me from death. The same with SH2, but I couldn't be bothered to kill in that, as they weren't that good at hitting you, so I ran past them all, even the bosses were easy to dodge.

It was still an enjoyable game for me, I just didn't want to fight, so I didn't. It actually made it even more tense, as I knew I had to face the enemies each time I went back into a corridor or room I'd previously visited.

The entire Fable Series. I have never died in Fable when I wasn't checking to see what the scars were like one my second playthrough. They're fucking ugly, by the way. The skin around the scar has the neutral skin colour, so if you're evil you'll have pale skin all over except for one massive patch on your face. If you you're good, and your skin is fairer then you look like you have vitiligo.

Plants Vs Zombies. The main campaign anyway. Still a lot of fun.
As mentioned, the entire Fable series.
Nearly did the first Assassins Creed, but died at the last battle.
Erm... Animal Crossing?

Fable 2

I didn't legitimately die at all during it. There was one time where I was fighting a boss and I wanted to kill them in an amusing way instead of what I had been doing so I decided to try again and figured the quickest way to do that would be to stand there and let myself die. Unfortunately that's when I found out you just stand right back up immediately after... Boy was that disappointing. Outside of that, I had massive amounts of useless potions I didn't need and never bought any extra. The severe lack of difficulty on that game was a real let down.

Silent Hill 2

Didn't quite finish it, but I came pretty close to the end. Eventually I just got really bored with it though. The story was cool, but the gameplay is easily the worst and most tedious of the original Silent Hill trilogy.

The Legend of Zelda: Majora's Mask

When I played through Majora's Mask I had just come off of marathoning Ocarina of Time (which I only had a couple of fluke deaths on anyway because it was rather easy), and at that point I had gotten into the swing of things enough to totally breeze through Majora's Mask even though it was harder by comparison. I'd also list Twilight Princess, but I haven't actually gotten around to completing that, but it's ridiculously easy.

Shadow of the Colossus

I've fallen off of them certainly and had to climb back up, but I have no idea what the game over screen looks like.

Eternal Sonata

Been a while, so this might be wrong... but I don't remember losing a single battle during the game when I played through it. There were some close calls, I've lost party members during fights, but I'm pretty sure I haven't lost any of the battles overall. I also was impatient, so I didn't grind much and was a bit under-leveled, which made some parts a real ***** as I had some of my party members die really easily which required multiple revives during certain parts.

The Legend of Dragoon

I didn't quite finish this one, but I played through 3/4 of it over a couple days and didn't die at all. I also had a number of spots where I would forget to save or just not even bother and I remember be relieved that I didn't have to redo a whole bunch of stuff again.

Black & White 1

I came pretty close to the end and dreaded being attacked by other gods/creatures the whole time, but during the entire thing it all went surprisingly smooth without any significant screw ups. Although, I did have a couple of luck out moments. (I especially dreaded it because strategy games are not my strong point.)

That's all I can think of offhand. There's others I'm sure, but I don't own them anymore, so it's harder to remember than simply thumbing over my collection. Other than that, there's other games I've played which I've never died on, but I only played halfway or so through them, so it wouldn't be fair to list and still had a good ways until the ending.

Also, for those who are wondering how I know how I know how far I was in some of these games I didn't finish since I hadn't seen the end. I've skimmed guides before to find out how much was left in the game.
