My surname has been changed twice.

The first time it was changed to my mothers maiden name when she and my father got divorced.
The second time it was changed to be both my fathers and my mothers surnames with a hyphen in between when they remarried.

Now that they are divorcing again and I am an adult, I think I will just keep it as it is for the time being.

I have never changed my name, I like Mark as a name, but I go by Markus and Marky a lot. Markus- because people always think I look more like a Markus than a Mark, and Marky because several lasses from my Uni days thought it sounded more playful & was fun to say, and it stuck with that group of mates.
So, never would change my name, but online I am normally Markus.

EDIT- Now that I think about it, I volunteered at a summer camp last year, and asked for Jamal on my name tag. That was a good week.

When I was about 11 I came up with the name 'Freaktwa' for no other reason than it would be something most people wouldn't already have as an account name. For some reason I stuck with the stupid name, even though people assumed that I meant to write Twat at the end. Upon getting older and realising it was fucking stupid, I came up with Professor Idle, and have used variations ever since.

Have never changed my name, online or not. Bartholen is my go-to internet forum name pretty much everywhere, sorta part of my identity now.

And the name derives from Bartholomew Roberts, the most succesful pirate of the Caribbean Sea.

I once changed my Steam Username to "Nullah" in reference to the God-Awful movie Australia. I did it just to reference the character "Going Walkabout" which people who saw the movie would recognize.I also Changed my name earlier to "Expresionless Dude" because I was going on a server to troll an old friend and he knew my normal name. I mainly just use "Toxinthegreat" or the only other variant that I've come up with so far "TSToxin1337". Yes, I know. 1337 is overused in gamertags. Real life name? I'm happy with mine.

nope, i like being Dan. not so keen on Daniel, always makes me feel like a 5 year old getting told off...

as for my online name, it has gone through a few changes, it started off as sergeantd (S.O.D. reference), then moved to cuppajoe (anthrax reference) and now Suitepee, or suitepee7 most of the time when i need a number (Suite-Pee being S.O.A.D.). the first few changes happened within a year, suitepee has remained for years now so pretty set on keeping it ^^

My real name is Martin and I quite like it.

I used to be ?[::.n4M3l3Ss.::]? on counterstrike when I was about 12 xD. I have RustlessCarrot on Xbox Live, while anything on pc is RustlessPotato. I don't know how I came up with that name. Some RL friends do call me Pothead (we used to play cs together).

I've changed my name online quite a few times, most recently about four years ago when I broke up with my ex and didn't really want him to have the option of cyberstalking me.

I've had about five different nicks on the internet during the 10 years I've spent here. Can't even remember half of them, but I've used Naeras since that name was one I randomly thought up when I named one of my alts in WoW(later to be my main and top 10 arena rated discpriest).

I also nagged my parents into giving me a middle name when I was about 5 years old, so I've technically changed names irl too. Not quite sure why I wanted it, I suspect it was just because my mom had one.

I never change my name in real life. Also it seems that Japanese names can be shortened to "chan" and "kun" (ex Maeda Atsuko = Acchan") but nothing like Michael = Mike, Joanne = Jo etc.
We also do not have middle names, so that is not a choice.

My English Game names are usually Evangelion related, but Japanese game names are totally random.

I'm going to need chemoradiation therapy after reading the OP.

Yes, I've changed my name before. Mainly because I had a name that made obscure pop culture references, but then realized that I should have a name that describes me rather than some piece of art someone else created that I appreciate.

Call it maturity, I guess.

Why?
I have several pals named Daniel and none of them thinks it's stupid o_O
It's a pretty common and easy name, no way special, not in good nor bad...

On Topic:

I have never changed a name online for an account but I have 2 or 3 different I use a bit depending on if the first one (Guffe) happens to be taken or can't be used for some reason.
Real name Niko and wouldn't change it, it's easy (except for those bastards who spell it Nico, a-holes >:0), short and easy. I think I have it because it was the name of my dads best friend in his childhood but I'm not completely sure.

No, but I would if I could. My psn tagname is hermes+. I had to introduce the number because Hermes alone is a used enough tag (besides a proper first name), but I would change it to a compound word if PSN left me do it.

I've only ever changed my online name once. I used to go by a pretty childish name I thought up in my early teens on every site I visited and by the time I was 18-ish it was getting kind of annoying. So, I either let the accounts die or changed the name. I'm rather this site gave the option, and when it was introduced I used up my one name change pretty damn sharpish.

Wouldn't change my real name though. Never really wanted or needed to.

It's odd, I've always used some variation of "Kyrian" (my first pen and paper rpg character) for a gamer tag. The only variation is if a "callsign" is more appropraite (first used in X vs TIE,) then its "Gambler" or a variant.

But my real name... I've tried to change it. It's unusual (nearly unique) and I hate getting asked questions about it. My middle name is Ryan and I tried to change it when I went to college. I chose a school that no one from my high school was going to and tried to leave it behind forever. It was fate... my roomate's name was Ryan, and there were 3 other Ryans on the same floor at the dorms... so I'm still just me.

And no, I don't ever use my real name online. When there's fewer than 10 or so of people with my name in the US, it makes me to easy to identify.

Well I'm many Shinsei Joutei-ish names but my LoL account was made before I learnt not to be an ass with it.
So my name was Jouteireaper...
When I saw the name change sale I jumped right in.
Now I go by Shinsei J and I feel much less smug.

I'm also thinking of changing my surname from just my fathers name to my mothers as well.
They're devorced and he's slowly cutting his ties to us and moving out of the country so my siblings and I are thinking about it.

Erja Perttu was what this girl nicknamed me at a camp one year, I was EP for a two weeks, which was awesome.

I used to want to change my real name, because no one can ever get the damn thing right unless they've known me a while and gotten it into their heads, but I grew into it.
