By a bad checkpoint I mean hitting the checkpoint in a game where you're in a pretty bad situation, eg. by a grenade or really low on health.

On MW2's Loose Ends mission on Veteran, I spawned right in the sights of an enemy sniper and died as soon as I respawned :(

there was one in Far cry that was in the middle of a battle between some of those big grunts with rocket launchers on their arms and some humans. it wasnt an instant kill but you could expect a rocket up your bum if you didnt get outta there fast.

One time in Fallout 3, I'd finished the Blood Ties quest and fast traveled to Arefu to get my XP reward from Evan King.

Weird thing happened: A Raider somehow spawned in Arefu. And no, this wasn't the weak Raider with maybe a sledgehammer or a 10mm SMG, it always spawned with a Combat Shotgun, Assault Rifle, or something else. It would murder Evan King with ease mere seconds after I'd arrive, then the game tell me I failed that quest, that I was now an enemy of everyone in Arefu and The Family, and that all the work that I did was for naught because the game decided to flip me the bird.

So, I spent literally the next ten minutes reloading the auto save when I fast travel to Arefu, taking pot shots at the Raider, and hoping that I would kill him faster than the Raider could kill Evan King.

Got the Raider eventually, but it was way too frustrating, and the next time I went to Arefu (to steal the Repair Bobblehead), ANOTHER Raider appeared and murdered Evan King. Fuck Arefu and fuck the Family, I don't give a fuck that he died because the game felt like spawning Raiders in a town.

One time in Call of Duty 1 I reached a checkpoint and was promptly killed by an artillery strike out of nowhere. The worst thing was I died literally seconds after the game saved, so there was nothing I could do but restart the level. Was close to the end of it too.

While I was trying to finish Black Ops on veteran.

During the "Rebirth" mission, you know the part when you're in the APC (the worst part in the entire game), well at one point I got a checkpoint that literally made it so that I was being shot at whenever I reloaded...

...many controllers were lost that day.

This used to happen all the time when I played Medal of Honour (the original, not the new one), because back then the series was still using health bars.

So I'd get to a checkpoint where I had 15 health, no ammo left in my machinegun, and dozens of pissed off Nazis between me and the submarine, or whatever I was supposed to blow up. I had two choices; restart the level, or attempt to forge ahead regardless.

AND I DID IT. Through fire, and steel, and the judicious application of grenades, I won the day!

A few years back me and my friend were playing halo 1, and he thought it would be funny to bash me in the back, and we got a checkpoint as soon as i died and he would kepp reloding it and i would keep dying.

Was a while back with Tomb Raider: Legend, and I didn't realize it at first.

It was the Bike chase in nepal. The checkpoints were on the narrow mountain side roads, and I jumped one corner with a checkpoint on it. Not that much of a problem, but I died in the following wider "battle" area, to be respawned at the checkpoint. Which was right above the chasm, but with no momentum to carry me to safety. So I plunged to death a few times before restarting the whole, tedious level.

Original CoD, when you're defending Pegasus bridge and need to get to the anti-tank gun. Checkpointed at the exact moment I walked next to a grenade. Tried to avoid it three or four times before I realized that I was too close.

portal still alive, threw a turret in the acid and fell in with it by accident, checkpoint point happened as i was in midair and kept respawning 2 seconds away from death.

does that qualify as a 'bad' checkpoint?

Many times when i had low health and none painkillers in Max Payne 1
"Ordinary Routine" 20HP left from the cop killing on the way there in mafia 1
RTCW, when you enter the underground of graveyard level, had 4 HP

That's more of a 'Oh shit I'm fucked' checkpoint...

Playing Half-Life 2 towards the end when you're fighting in City 17. I remember accidentally falling down a destroyed staircase you need to jump over. It check pointed mid fall. Every time the check point loaded I was falling to my death.

Call of duty: black ops, veteran difficultly.

During the mission where you have to sneak into the missile base and shoot down the rocket shortly after it's launch. My checkpoint of sorrow happened about halfway through the level, in a large room with two corridors leading away on the far sides of the room. Check point catches me with my secondary having an empty clip, and my primary (an ak-47)only having two rounds left in the clip. Theres two guys behind me, taking pot shots at me, 3 more in front of me, shooting at me and my incompetent ai team mates, one pretty much next to me in knife range, and more coming from the far corridor.
The kicker? I was already half dead.

Took me a long time to get past that checkpoint.

Fallout New Vegas, my first save in 4 hours was standing next to a beeping mine that resulted in me clipping into the side of a mountain, or, whilst we're on Bethesda, during Oblivion, saving at the end of the Thieves Guild final quest, just before a big stone goliath jumps me.

My cat didn't come into my room for weeks after my cry of anguish.
