Not in a car.
I've run out of gas on a bike, but that's basically a rite of passage in motorcycling. Most of them don't even have fuel gauges.

Once, literally as I turned into a gas station.
As in, no more gas, coasted in next to an empty spot next to a pump.

I ran out of gas once because my fuel gauge was broken, and I didn't know it was. Luckily I had a AAA membership.
Also, most vehicle manufacturers now set the fuel gauge so that it reads empty when you have roughly 50 miles left before it is really empty.

Before I bought my car, it was t-boned, and I guess the damage broke the low gas light.
Thankfully the one time I ran out of gas, it was in my own driveway. I got home and parked it one day, and the next morning it simply wouldn't start. Boy did I feel stupid.

I was once. I was about a mile from the gas station and had to run there. It was not fun but for some reason I didn't learn my lesson because I still drive around on E.
Never, my car has only ever gotten down to a little below the halfway mark. I am always on top of things like that.

OT: I've never run out, but I think I've had some close calls. See, I'm kind of OCD with gas stations. I have a Petro Points card and I ONLY ever fill up at Petro Canadas. This became a bit of a challenge when I was driving across the country and there was only one gas station every 400 clicks... I pretty much almost had to plan my route based on Petro Canada locations.
Nope. Though I've come close to it on multiple occasions. I wait until that light goes on every time because I hate stopping to get gas.
I did once with my mom because the fuel gauge was apparently broken and stuck on 1/4 of a tank. Didn't find out that broken fuel gauge part until later.
