I'd like to be on by death bed, taken up in an airplane, then pushed out. Because the only bad thing about falling is hitting the ground; the rest is awesome. Since I'd be dead in a couple days at most anyway, might as well go out in style; falling out of an airplane.
Saving a puppy from a burning car with one hand and pushing someone out of the path of a speeding train with the other.
While I'm asleep, just randomly one night without warning. Completely unexpected, like, not when I'm old and going to die anyway. As in it could happen tonight, if it wanted to.
Either that, or in a giant earth shattering devastating disaster. Some atomic explosion or giant meteor, and I'm really close to the impact point. That way, I don't live long enough to feel any pain or after effects, but I still get a nice fireworks display for half a second. That would be fun.

By breaking almost every bone in my body after falling into a shallow vat of sulfuric acid which I will land face first in which will put out the fire covering my body after a horrific acupuncture experiment which actually involved electrified nails left me aflame when the car battery sparked and lit the kerosine that'd had been tossed over me by an angry gypsy after he gouged my eyes out with a spoon.....Oh wait, was it how we wanted to die or what is most likely to happen to us when we die. How I wish to die.......falling from orbit in a space suit. The pain from re-entry will be fearsome, but the view will be fantastic right up until I fall right on to the tip of the Washington monument for one last show for the bystanders.
Being serious:I would Want to die in my sleep. Atleast there is no pain involved.
Not being serious:In A crowded Street, Sandwich in one hand, Knife in the other. After I engage the crazy drunk Businessman In a knifefight, he stabs me and I die. To tie it all together I would have to scream "Peanut Covered elephant nuts"!!!. It confuses the hell out of the murderer And I die with a smile

Making a stand for something greater than my own single life. When I go out, I want to it to be with a mountain of my foes around me, broken and scattered. I want a death that will be talked about for centuries after the fact.
So, if I can't have that, I plan to live forever out of pure spite. >.>
