Would Eternal Darkness: Sanity's Requiem count?
The way some of the protagonists die is kindof horrific, most of them trapped that way for centuries. I think one guy got plastered into a tower made of people.
Mad Max Roivas's autospies are more lightly disturbing sounds than imagery, though.

Well if you want fucked up then you might like Song of Saya (Saya no Uta). It's a visual novel, but it has no shortage of disturbing-as-fuck imagery.

Christ that game...

Goddamn, there are a lot of Suggestions for Song of Saya. So many that I got it and played the whole thing. Jesus Christ. I loved it. It was great. One thing though. One little tiny thing that could have been mentioned about this Japanese game.

I expect there to be sex scenes in the more mature visual novels. Someone could have been kind enough to mention what KIND of scenes there were. Kinda wasn't prepared for THAT specifically.

But yeah, that was pretty disturbing! And I loved it! The doc went out like a goddamn CHAMPION.

Someone mentioned it earlier but I think it deserves repeating.

Cryostasis: The Sleep or Reason.

A lost gem of a game that I think will scratch your itch.

It's a survival horror game with a pretty unique feel. It's disturbing though not particularly gory or graphic (just creepy). It doesn't ramp up the fear too much either and you can fight back quite well. It's highly atmospheric.

Unfortunately I have no clue where you can get it. It's rare to find it in stores now and it's not on Steam or GoG. But if you can get it, it's quite the experience. It has a few problems though, so go in expecting a flawed, slightly cracked gem.

I'll place another vote for Saya no Uta, Eternal Darkness: Sanity's Requiem, and all of the Megaten games...

Also, I'll add in the game Catherine into the pile, where it apparently takes place within the same universe as Megaten's Persona games...

Seeing as how Saya no Uta has been mentioned already a gazillion times, I'll add a +1 to Lone Survivor. It's a 2D side-scrolling RPG that kills it with atmosphere. It's much tamer than most of the games here but it still has a decent amount of disturbing imagery. Overall, a really neat and eerie game with a soundtrack that proves Jasper Byrne really knows his shit.
