For really good customization anything by Perfect World would probably be up your alley.

I was going to suggest Dungeon Fighter Online or Dragon Nest until you said that. I'd keep them in mind just in case though. :D

Edit: Forget what I said, Bobic's right. How'd I forget that one?

I've been having a lot of fun with Age of Conan lately. I'd recommend that! It's got a nice engine and good gameplay (although it's a bit more difficult than usual MMO fare, I've found).

Either LotRO or DDO.

The former is a more traditional MMO, take turns kicking a mob in the shins, get XP, heal and repeat. And, some quests.

The latter, on the other hand, the towns are merely hubs for quests, which are all private instances. Thus, you don't really have anyone camping a specific spawn. Plus, since you get the XP at the end of the instance, you get more of a sense of accomplishment, rather than just killing the same Kobold for the thousandth time.

Yes, they both have shop for micro-transactions, but that currency can be earned by playing in game.
You get TP (Turbine's currency) though earning deeds and what not.

For example;
Kill 30 wolves in the shire? Get 5 TP. Kill another 60? Get 10 TP.
Complete 15 quests in the Shire? Get 5 TP. Complete 30 quests? Get another 10 TP. Complete 45 quests? Get another 15 TP.
Do ability [X] 75 times? Get 5 TP.
Completing quests earns you favor. For every 100 total favor you get on a character, you get 25 TP. Also, there are benchmarks of total favor to get you extra TP for the first character who reaches them on a server.

The visual character customization is decent, it's not great but has good enough diversity that you probably won't find someone that looks exactly like you. I found someone who looked remarkably similar to my character only once. We took a screenshot, laughed at it and went our separate ways, haven't seen him since.
As far as classes.... well there are no classes, just skills. Aside from a few that are race specific, all skills (somewhere around 90 of them spread over Combat, Magic, Alchemy, and Life skills) are learnable by any character.

Dragon Nest its put out by Nexon who does fairly solid stuff. Its a free to play mmorpg that is a action rpg more than a WoW click talents to see what happens grind fest. Its in beta at the moment and if you play it now you get a few cool freebies that carry over when the game launches in a few weeks.

there's no such thing, they all exist to parasitically draw your money from your bank account for no good reason

go play a straight rpg instead, it's a far superior gaming experience because the development is spent on game-play instead of repeatable quests and seventeen septillion hours of grind

that said, the most enjoyable experience i've had with an mmo is aion because once you get past about level 30 you get really cool dungeons to explore and you can solo them which is far more fun than other games, there is of-course mass pvp and group dungeons as well

I would suggest Guild Wars 2 when it comes out. Although it's not free, you only have to buy the game itself, there are no subscription fees(sorry if the non-free part really bothers you). But at the same time it should have really good character customization, in terms of appearance and playstyle.

Even if having to buy it discourages you, it's worth at least looking it up for the sheer amount of changes ArenaNet is making to the MMORPG genre.

The three main changes are the personal storylines, dynamic events and non static combat. They are doing away with quests and grinding, replacing them with a dynamic event system.

Combat is also going to be dynamic, faster paced, and more skill-based(especially their structured pvp). Plus there's a personal storyline, where you can make choices.(the results are mostly reflected in the rest of the story and your own home instance.)

Each individual profession(the classes) are based more on different playstyles than the holy trinity roles. Each role is being replaced with their own broader terms. (damage vs DPS, control vs tank, support vs healer). Control being instead of just standing in the way of the enemy and taking hits, you can stop them by crippling them or block their movement with wards. Support can be protecting allies with barriers, or giving them a variety of buffs(known as boons in game) and healing being only a very small portion of ways to help your team. Damage is more than just trying to dish out as much damage in as little time as possible, it expands to things like slower wide spread havoc or burning them; and many professions can use the same type of weapons, but have unique uses to them when compared to other professions.

I would say based on what they're doing with this game it would be well worth your money.

Just a little warning if you play DDO, if you havn't played D&D yet, I would recommend you use a premade build for a class.

As an odd one from all the others, I'd suggest a lesser known SciFi MMORPG, Black Prophecy.
It's free, has quite a bit of content, and has a story to go with it. And typically free games are worth at least checking out once :p
