Honestly I think the precision of the mouse and keyboard set up have yet to be duplicated in any form of alternative gaming controller. So for me it would have to be a refinement of those. Perhaps a keypad for one hand and a mouse or track ball for the other.
Oh sure you don't hold it as one unit for both hands, but when you want precision you need more precise leverage.
Its the same reason that tournament games use sticks with big boxes under them. You don't hold them you rest them on your lap. The same could be done here and the precision would be worth it IMO.
Edit: Well, I suppose you could always make some refined version of what they use in targeting for helicopters and jets. A piece of headwear than scans where you are looking with lasers. Its a bit of a technological and marketing challenge though.
