Everyone has in one way or another tbh.

I mean some are pretty great but they still brought me "why?! ...WHY?!?!?" moments, so I'd say... no there is not a developer who has never failed me.

Black Isle, Blizzard, Microprose, Bethesda (but only for their Daggerfall games) and Bioware

Valve has failed me by fucking with my good steam account ._. (If thats valves fault and not steams I dunno)
SEGA failed with Sonic... He does not need a sword or to become a werewolf! >.<
Bethesda failed you with oblivion??? Any game capable of robing me of 140 hours of my life including dlc is not very disapointing. I know the imersion could have been better but still how can you hate it.

Paradox Interactive, Ascaron (Daniel Dumont and Bernd Ludewig, who are now with Kalypso Media), Creative Assembly, Sports Interactive, and Out of the Park Developments leap to mind.

Lesson? Finding a niche and being consistently awesome at it is a good way to avoid hate from your fans.

I'm gonna go ahead and say Infinity ward for COD, COD2, COD:MW, COD:MW2 and Cod and chips.

Also Bethesda and Bioware

Hmm. Well, I can think of plenty of companies that have never failed my expectations, largely because I never had high expectations.

If I assumed that "failing me" included expecting good games, Team Ico would be the only one to never do so: both their games were elegantly refined and beautiful. Black Isle would be the second closest, before they shut down. Troika released several buggy, unfinished games (that is, all of its games); Blizzard's games are solid, but don't innovate to a large degree (and innovate very little outside of game mechanics); Bay 12 Games's development times are long (but understandably so because of the load of features being added).

Don't look at DoW2 (dammit relic, you were the only significant PC dev who hadn't fallen).

I can't think of a Konami title I haven't loved, though I also can't think of one that wasn't a metal gear game. Double fine haven't yet, though I only have two double fine games, so I doubt that counts.
