It's the first shot of the movie!

The movie is about the end of the world. The nukes have fallen, the nations have collapsed, all that. The main character and his sidekick are both trying to start up a hospital. They attempt to repair a damaged hospital, make medical tools, all that! The first shot of the movie is about them getting the idea, and doing the very early stages of it.

What would be the first shot of the movie?

A mushroom cloud, silly.

The first thing you hear is a high-pitched continuous sound.
A green line appears from black and stays there with the background changing to a darker green.
The Camera zooms back and the viewer sees a still function ECG that is dangling halfway out of a drawer.
he Camera continues zooming and it is revealed that the ECG belongs to an ambulance that crashed. (as the camera zooms out the tone lessens more and more).
The Camera comes to a stop about 15 feet from the back of the ambulance (and about 6 inches off the ground)looking back inside the ambulance. (the camera basically zoomed from that point).
About 1 foot and 45 degrees to the left of the cameras position, a rolled up bandage lies on the ground.
The ECG sound has become nearly silent.
We now hear the steps (shoes on gravel) coming from behind the camera.
A tattered but wearable shoe appears left of the camera and a hand grabs the bandage.
We hear a near jubilant "You were right" that seems to belong to (insert sidekicks age, gender here).
More footsteps, this time running away and to the back of the camera.
They come back with another pair of footsteps that seem to drag (one foot drags along the ground).
Protagonist and Sidekick come in the camera's view from the right and take seat on the edge of the ambulance.
Sidekick takes out a knife to cut away the dirty makeshift bandage around the Protagonist's thigh.
Before he starts cutting though he says: "Sorry wait a second, this thing is annoying" (sound slightly muted due to distance) climbs into the ambulance and switches off the ECG.
He sits down again and starts cutting the bandage.
Fade to black.
