I'm not aware of any major Windows 7 related issues. Well...there is like one but even then it's so bizarre and non-replicable I don't even know if it is the OS. Pretty rare, though - I've heard of it two or maybe three times.
Anyway, you just need the Unofficial Patch [http://www.patches-scrolls.de/vampire_bloodlines.php] and you should be good to go. Now, here is a small catch - make sure you turn off the Steam updates if you have Blooldines on Steam. If you have the retail version, try actually patching with the official 1.2 first and then install the UP.
There is a small issue with applying the UP straight onto the vanilla game (Steam comes prepatched to 1.2) so hopefully that would fix it. If you do get an error saying something like "no precache: models/character/npc/common/security_guard/security_guard_skinny/security_guard_skinny.mdl" (that's most likely going to be well into the game after you save Johansen, so it's near the end), then go to the \Vampire\dlls\ folder and try renaming one of the extra files into
vampire.dll. Alternatively, just give me a shout if you feel lost, I'll walk you through it.
So, to recap - install the UP and the game should run. The only "problem" with Windows 7 I can think of is that you won't see the intros - you can still hear them and cancel them, though. I doubt that's a huge issue, as the intros are an ad for nVidia, Activision's logo, White Wolf's logo and one more thing. If you really do feel you miss out, though, you can run the game in compatibility mode for Windows XP and that's going to fix them.
