Spider-Man 2: The Movie The Game, there was a level where you fought aliens, there were no aliens in Spiderman 2! The game developers pulled that straight out of their asses...

Yeah, plus the title that's pretty much it. My personal favorite difference though is the character 'Dizzy' Gilespy(sp?). In the movie the character was a hot redhead that ends up being a romantic interest for Johnny. In the book...a guy who dies on the first page.
You do realize that Starship troopers, or "Bug Hunt at Outpost Nine" wasn't an actual adaption of the book. The film had already entered production when someone stood up and said "hey your movie is similar to this book". And thus they took the name, and a couple character's names, and smashed it together with their movie and a misunderstood bastard child was born.

This. What about Mario Bros. screams "Englishman and Latino fighting Dennis Hopper and his army of 8ft Goombas in a Blade Runner style fucked up dystopia"?

Were they even brothers in this film? It's almost as if they just cynically took loads of names from the series and applied them to some bland sci-fi in orer to boost it's appeal... but the movie industry would never do that, right?

BloodRayne. If I hadn't seen it I wouldn't believe it was possible to make a film I didn't love out of that game.

Most adaption do tend to suck in some way compared to their original, but some a lot more than others.

I'm gonna say V for Vendetta, the setting is very different in the comic and the ending to it is not a happy one like the movie.
