Least favorite would be Eiken or Bokatsu Tenshi Dokuro-chan. Dishonorable mentioned of Naruto, not because of the show itself (though 85 straight episodes of filler is bad), but because people use it to bash anime.
Favorites would be, hm... Gurren Lagann, Majin Tantei Nougami Neuro, Eyeshield 21, oh, and Axis Powers Hetalia. (IcannotbelieveIalmostforgotthatone) There are plenty others, but those are the first that come to mind.
Least favorite is Blue Dragon. I still can't believe I read the whole thing, but at least it was illustrated by Obata Takeshi. D:

Anime
Most: Ninnin ga Shinobuden (Ninja Nonsense). Absolutely hilarous, and one of Norio Wakamoto's better roles (which is saying something). Also, I'm still hung up on Nanoha (first season) and Princess Tutu.
Least: Hard to say, because I avoid most anime. Probably either Tenchi in Tokyo or Deathnote (for different reasons: the first one sucked, I just couldn't get into the second).
Manga
Most: Fruits Basket and Negima, I can't decide between the two. I'll also put a good word in for Hayate the Combat Butler and Ai Yori Aoshi.
Least: A number of yaoi titles with non-consensual sex, and silly plots besides. For recognizability...the only purchase I've ever regretted was Yugioh (I have the seven pre-Duel Monsters volumes), which gave me nightmares.

Favorite are Spice and Wolf, Gunslinger Girl, Code Geass, Mobile Suit Gundam, and Final Fantasy Unlimited. Manga wise I am reading Pokemon Adventure which is a hell of a lot better than the anime.
