Little things that developers overlook

This is a semi-response to the resent Extra Punctuation article.

This differs from previous threads about this kind of subject, which usually ask about the sort of little things you appreciate. I'm in fact asking what little things in a game, little picky things that you may be considered OCD or pedantic to point out, but may make quite a big difference to the depth or aesthetics of a game. But with actual examples that developers have actually overlooked.

My example is the clothing in Oblivion. Apart from the armour, the clothing just looks like all the characters are wearing pyjamas. It made me surprisingly frustrated. Not to mention that all the characters are wearing the in-game world generic outfits. Which I often see in games. I find it just quite lazy, well, as lazy as a game developer can be (I still admire developers).

So, what about you?

Characters with natural human emotions, I can't think of one game off the top of my head that realistically depicts how a human would react.

Any game that pretends to be at least somewhat realistic but then puts in copious amounts of bullshit. The recent Call of Duty games are fantastic examples of this. I hope I shouldn't have to explain why.

I guess this is really not a small thing nor is it a specific example, so I'll just go ahead and say ammo pools. Please, give me individual magazines. It's not hard to implement. (Note: This depends on the game. Ammo pools may be fine in Halo, but they should not be in Operation Flashpoint)
