Try Samurai Champloo. No needless gore, elegant sexual scenes, great and mostly realistic swordfights, only one WTF episode out of 25 and one of the greatest endings that I have seen so far.
Oh, yes, it's about samurais in the Edo period and the soundtrack is made mostly of hip-hop, but as a rock fan I can assure you that it is really nice hip-hop.
In short, you will like it.

Really? No one (I skimmed it, forgive me if I'm wrong) has mentioned Fullmetal Alchemist or Death Note? Easily my favorite two animes, and they're both easy to find dubs of, so if subs aren't your thing you're in luck.

- Yes, I pretty much only watch anime that runs on Adult Swim (other than Hellsing, of course!)

I've recommended over and over and I'm going to recommend it again: Gurren F****'n Lagann. Possibly the most epic show you could ever see. With also 1 WTF type episode, (i think it might be episode 6, so be advised)it's at least in my opinion, the best thing i've watched so far next to Cowboy Bebop.

You may enjoy the second Fullmetal Alchemist anime (Brotherhood). It's grounded, in its own way, and the giant conspiracy is very well done.

I'll second Record of Lodoss War - just the main series though, the spin-offs aren't so great. I would further suggest Spice and Wolf, Planetes, and Eve no Jikan - although none of those three have the "cool fight scenes" you asked for, they are nonetheless very, very good.

I'd also suggest an older, slightly odd, giant mecha anime called Gunbuster - It changes theme over it's six episodes from Rocky to Top Gun to The Forever War. I'd say it has pretty good fight scenes, although of the mecha variety, not human-on-human.

It doesn't really have fight scenes, but I really liked Death Note, and I'm not a huge anime fan either. I haven't watched Code Geass but I hear it's quite similar to Death Note.

I absolutely loved Gurren Lagann, but it's not the best recommendation in this situation. The OP is clearly not a Gainax fan, so we need to aim for a less ludicrous studio.

You don't hate most anime, you hate the stereotypical shounen anime.

OT: I'd recommend Bokurano. It's very psychological while still being very straight forward and emotional, and it has some great mecha fights.

Haha.. I was mostly concerned because it was at the top of the list. You could have at least given a nice recommendation before going that direction (not that they're bad series, I loved the better part of your list).

For a grown-up anime (and by that, I mean you can handle grim stuff), I'd recommend Black Lagoon. It's a Mad House production so the animation is excellent, it's a modern-day story so there's no fantasy or sci-fi, and the story is character-driven and pretty deep sometimes. However, I skip the second season's Vampire Twins eps as they're a) disturbing - VERY disturbing, and b) just not as believably done. They are good from a story point, so that's up to you.

Regarding endings, that show doesn't really. It's still progressing but the manga is coming along slowly, so it'll be awhile before a third season.

I'd also recommend Full Metal Alchemist / FMA Brotherhood. They're both good in different ways, although for me the endings of both were not to my taste. But great story and gotta love the characters.

Others I like include Cowboy Bebop (best depiction of a hand-to-hand fight scene ever in the first episode), Ghost in the Shell Stand Alone Complex 1, Last Exile, Claymore (although my teeth ache to re-edit it for pacing), and Moribito - Guardian of the Spirit.

Oh, and subs. ALWAYS subs. Cowboy Bebop had the only dub I found watchable, and it was still only 3/4 as good as the original voices. Everything else, not so much.

I will third the GITS suggestion, as it's probably the best "serious" anime I've seen. Aside from that, Gundam Wing is excellent, lost of political machinations/intrigue/things like that, and above all else, amazing fights between giant mech suits.

I will also recommend Afro Samurai--it's more of a miniseries, but it's excellently donw and brings up quite a few poignant observations about the human condidtion. There's a brief sex scene in the 3rd episode, tastefully done but definitely a little more than what most Americans are used to.

Others you might consider:
Trigun
Cowboy Bebop
Samurai Champloo
Shaman King (a little long, 65 episodes, and maybe a little more on the "kid" side than you might like, but great fights and some pretty good humor.)

I think I might be misunderstanding the question completely because after looking at some of the replies, animes like Ghost in the Shell and Elfen Lied pretty much fall under what I THINK he/she was trying to avoid. GITS is pretty much sci-fi stuff (Not down to Earth) and Elfen Lied is reaaaaally gory if you're not used to that kind of stuff.

Cowboy Bebop, Fullmetal Alchemist, Death Note; they're all not very down to Earth.

But then I noticed he/she liked Code Geass so...that's where I'm confused.

I suppose my own choices would be:

-Fullmetal Alchemist (Though it's kinda lengthy but not as bad as, let's say, One Piece or freaking DBZ?): Interesting story with original protagonists. it's nice to see an anime that uses a new breed of heroes and makes them work.

-Death Note: Kinda like a huge Cat and mouse game. It's not that long, honestly, and it will always keep you guessing what's gonna happen next. It almost makes you root for the bad guy. It did that for me.

-Cowboy bebop: Haven't seen much of it but what I've seen was funny, interesting and not boring. So there's that.

Should I even go as far as to recommend RomeoxJuliet? As you might have figured out, that anime is about Romeo and Juliet in some sort of fantasy setting and you have most of the characters from the original play, even Shakespeare (?). It's also pretty short, interesting but, and be warned, super corny. I mean, it's a love story, you know. But some cool actions scenes make up for it.

EDIT: Shaman King is good but it kinda gets confusing towards the end. At least it confused me. But it was good.

Well Serial Experiments Lain is my most favorite show 3v4r... I own it on every format it's been released including the special edition, limited released blu ray (imported that thing from Japan...) plus a mishmash of other things, like, art books, t shirts, hats, pins, and statuettes (Their not dolls!) just to name a few...<.<

I'm still looking to buy the game though...
