Have you heard of "I Am Alive"? you're description reminded me of the trailer for that game (except it doesn't have monsters...though, if it makes a profit it gives reason to hope for a version with monsters). I've been wanting a game like that since I played "Disaster Report". "DR" you have to find clean water, navigate through a earthquake ravished island, dodge crashing buildings, and stuff like that.

I presume that if you are a psychopath fighting while be more like in condemned you make the player fell the force of the hit to show how brutal it is and when you are trying to kill a group of people in a building you can be creative like cut the phone lines then steal all the keys and lock the doors so they can escape then cut the power and then slowly pick them off one by one in cruel ways for cheap joke like hanging someone and then wright in there blood "I'm just hanging around" see one of my previous post for another cruel way I would kill someone in a game where you play a psychopath.
Um...well, you are creative. To quote the Joker from "The Dark Knight" "Why so serious?" (I haven't heard anybody make that reference in a while.)

If I were to design a horror game, I'd design it based off of Myst. Minimal/No contact with others, only traces that they were once there. This includes whatever monster you'd be up against. Try and make it as atmospheric as possible, make the game about escaping via puzzles instead of fighting via FPS like most current horror games I've seen.

Whenever you try to use an item, it becomes diamonds. You then have to reassemble the item with the diamonds, while being chased by samurai zombies.
