No one died on my SECOND go. I didn't have Miranda's loyalty or Zaeed's loyalty, but in order to save them I decided not to use them on the last mission.

On my first go I used Jack during my final mission and used Grunt as an escort, Miranda died. I thought fine. Good. I didn't like her that much anyway... But the game got to me. So I loaded up a save before the main mission and changed the escort to Mordin due to other forum posts saying that Mordin is the best for the escort, and decided that because of the tension between Jack and Miranda I thought that I would choose someone else for the final team.

That did it. I saved everyone. 34 hours of careful gameplay and I was pretty satisfied. The ending was a bit of a cliche and running around talking to your fellow ship mates after the battle didn't yield any great results, but I wasn't expecting the ending to be great or the aftermath.

I'm expecting that ME3 would serve it. And it better, otherwise I won't be so forgiving.

Note also that i'm not regretting my decision to romance Tali. I was about as attached to her as It is possible to be attached to a videogame character and still be normal. If anything, I'm disappointed in the game for not creating an adequate resolution to the relationship.
Did you not talk to her after the collector's ship?

that basically she's not feeling well, and it was totally worth it and really likes you still

though not perfect, I thought it was still enough to make me look forward to continuing it in ME3 (is that possible for people who loaded their ME1 characters?)
Yeah, I talked to her. I'm glad that they at least gave us that 1 line, but after that if you try talking to her, she still talks as if you haven't slept with her yet. They just leave it hanging flat. Oh well. Maybe they'll give it some more depth in ME3

No, i'd say it is because that Tali is a girl that doesnt think too much of herself and how much you care about her, thinking that you'd want to pursue other women because she was just a simple quarian. She always second guesses herself thinking herself unworthy of Commander Sheppard.

Thats why i like talking to her after the game is over, to assure her that i would never leave her :)

I've lost Thane(tunnels) and Zaeed(Distraction) my first play through, which didnt hurt with Thane but i really regretted losing Zaeed. 2nd playthrough: just jacod to the tunnels. didnt care, but also felt closer to Thane that time around because i talked to him more.

On my first run through the end, everyone managed to live except Thane and half of Normandy's crew. I had Thane go through the vents, and everything was fine until he was closing the door and then stuck his head out and got shoot through a hole about three inches thick. Miranda led both strike teams, and Garrus and Tali were my teammates the whole time. Samara held the biotic field up.

Like I said, everyone else lived. I apparently didn't get there in time though and I lost Yeoman, Gabby, and some other people who I didn't care about. Then I did the mission over because Thane wasn't badass enough to be the only death on my team. I just swapped him out for Tali, who unjammed the doors without having to pull. No deaths.

I managed to keep everyone my fist time through

Vents- Legion
Team 2- Garrus
Bubble shield- Samara
Escort- Jacob
Final- Legon and Thane

Romance- Thane (im a sucker for green)

Blew up the ship

But when i heard that every decision had a consequence i purposely made 2 games and each had opposite choices, so next time through ill save the ship and see what happens

Amazing ending, but...argh!

Upgraded the ship, no problems there, bar a bumpy landing.
Made Tali go through the vents (seem to be the only person to have done so...), and that was fine. Garrus was second squad leader and he owned that shit, of course.
Saved the crew, minus some random red shirt, and Jacob escorted them back fine.

Made the bubble shield Samara, and that was fine, but unfortunately my diversion team or whatever it was had Zaeed in charge, and he got ganked getting through the blast doors. Little annoying as no achievement for me, but not so bad as I didn't like him..

Then on the way back to the ship it just shows me Mordins dead body. So pissed about that, as I loved his character and his Gilbert and Sullivan ; Salarian Scientist.

Everyone else survived, had all loyalties (I maxed out paragon waaaay early and had no trouble with Jack/Miranda). My main worry was that Thane, or more importantly, Garrus would die. Thank. Fking. God he didn't. I'd have cried.

Saved the base, which might be interesting seeing as most people destroyed it, it seems..

I chose Garrus to lead both of the fire teams, and I have Tali go through the vents. For the first part I had Jack and Grunt with me, and we steam rolled through stuff. Everyone survived that part. For the next portion I had Garrus lead the fire team again, and I had Samara hold the barrier while Jack, Grunt and I ran through the place no problem. For the last fight I took Jack and Mordin with me, because I really didn't want Mordin to die, and I read up that he was one of the squishiest ones. Everyone lived through my mission, and I was happy. OH, right, I had everyone's loyalties except Miranda and Zaeed's, and I had Legion run back with the crew. 0 casualties. Achievement attained.. I was... So happy. ;3 Btw, I sexed up Jack.. I don't care what you say, that romance is far more rewarding.

First is the ship upgrades, fairly simple, there are three key ones, armour, shields and weapons. For each one you don't upgrade, a character will die.

For the first part of the mission, you need a loyal tech expert in the vents and a good loyal leader for the second team. If you pick someone is isn't a tech expert for the vents, or pick a bad leader, the vents guy will die.

For the bubble you need a loyal expert biotic.
If you pick a standard biotic, or pick an unloyal expert, then the bubble will fail before the end and you well lose people.

Again, the second team needs a good leader, pick a bad leader the second time and the leader themselves dies.

For taking the crew back to the ship you need someone loyal, no other factors matter.

The last bit is where most people get confused. The team "holding the line" needs to have good defenders, and they need to be loyal. If you take some of your best defenders, for example Grunt, with you to the final boss then they won't be there to hold the line and other people will die. The one who dies the most here is Mordin, because he is the worst defender on the team. One way to avoid this is to take Mordin with you to the boss.

And the very final bit is the run back to the ship, if the two people on your team for the boss aren't loyal, then they can die on the run back to the ship.

Well,

Jacob was in the vents...came back shot in the head, and thank GOD! He was such a bland character.

Thane got blow to shit when the ship took a few direct hits.

Mordin bled out whilst he made a diversion, and I like him too, I felt an emotiontal connection....I am the very model of a Salarian Scientist.

Wanted Miranda to die too, my romance was w/ Tali, and NO FACE! Pissed me off sooooo much, but she had to pretty to shepard wouldn't hit that shit.

No romance....
Legion and Grunt all the way. Survived it all.
Zaheed ran the fire team ..survived
Tali died trying to close the door.
Zaheed died leading the second split up.
Samara as the bug zapper.
No one sent with the Doctor and Kelly because I'm a Renegade bastard.
Didn't blow up the collector base because I'm a Renegade bastard.

Watched a great scene with Legion on Tali's Migrant Fleet.
Wrex survived and Ashley died. Full Renegade in ME1.

Replayed ME1 and replaying ME2 with:
full Paragon, Wrex surviving, Kaiden dead, the Blue alien chick as my love

I did as well. Just don't ask me where the last two days went. Also, I don't know how it happened, but one of my party member died.
Samara was taken by the swarm in the telekinetic bubble tunnel.

Also, I was male, so I chose Tali as my love interest. And I don't regret it at all.
Same here. Too bad my crew died, but at leats my team lived. Now I just have to find all the side missions and get some more upgrades.

I let Jacob volunteer to go in the tube and lost him right off the bat. Samara lead team two. I tried again, with Tali in the tube and lost her the same way. I couldn't reload the first save fast enough. Jacob wants to eat a rocket, Jacob is welcome to it!

Got to the survivors in time, had Mordin lead them back (thinking they could use a doctor).

Samara held the biotic bubble, Garrus lead team two. Garrus scared me by taking a shot to the gut, but shook it off. That's my man!

Killed the monster with Garrus and Thane at my side, lost no one else. Blew the base up, told off the Illusive man, desperately looking forward to ME3. Loved every minute of this one!

I looked up online later how to save Jacob, but I feel like loosing him was what I got and I'm going forward with my game as I played it without having to look up answers.

Plus, Jacob was the only character I disliked, so he can bugger off and hang with Ashley forever in the graveyard of annoying companions. XD

I know right! I've had a little while to reflect on that, and I'm starting to think that maybe they did that to screw with anybody who just got with her to see what quarians look like.

I'm pretty sure you have to have tech, biotics, and damage reduction maxed out, along with picking the right people and everything else for your whole team to survive.

For my second playthrough:

I was playing as a female Sentinel, and Garrus and Legion were my squadmates. I was trying to get the No One Left Behind achievements, so I was very careful for who I picked for the sub-missions.

I chose Tali for the hacking sub-mission.

I chose Miranda for the second fire team.

I chose Subject Zero as the biotic bubble maker to protect me, Garrus, and Legion from the seeker swarms.

I chose Miranda for the second fire team again, nearly had a heart attack when she got shot, but lived, and sent Mordin to escort Chakwas and friends back to the ship.

I made it through the mission without much trouble, and without anyone dying, and after the Reaper was destroyed, Martin Sheen demanded I keep the base. I was tempted to for the sheer Renegade factor, but remembered why it was a stupid idea and blew it all up.

Nearly had a heart attack again when Garrus was falling off the ship, but saved him and got off the station with everybody alive just before it blew up, flying off into the proverbial sunset.

Everyone was loyal, I didn't romance anyone and I had fully upgraded the ship;
Miranda led the first Fire Team: survived
Tali went into the vents: survived
Samara held the biotic bubble: survived
Grunt led the second fire team: died
Garrus escorted the crew back to the ship: survived
Mordin died at the end for no reason at all.
Kept the Reaper ship because it could be used against the Reapers.
